斑秃患者血清维生素D水平及幽门螺杆菌感染情况的探讨

摘　　要：目的:了解幽门螺杆菌(helicobacter pylori,Hp)感染及25(OH)维生素D[25-(OH)-D]的血清学水平与斑秃(alopecia areata,AA)的相关性,并探讨其临床意义。方法:采用^(13)碳尿素呼气实验(^(13)C-UBT)检测斑秃患者胃肠道内的幽门螺杆菌的感染情况。采用电化学发光法测定斑秃患者外周血中25-(0H)-D水平。结果:斑秃患者组与健康对照组的Hp感染情况无统计学差异(P>0.05)。同样,该感染与斑秃是否活动、患者的病程及斑秃的严重程度也都未发现明显相关性。血清25-(OH)-D检测发现,斑秃患者的水平明显低于健康对照组,而且,病程<1年的患者25-(OH)-D水平明显低于病程>1年的患者。结论:Hp感染与斑秃的发病未发现明显相关性。维生素D的缺乏与斑秃有明显相关性。Objective： To investigate the infection of helicobacter pylori and the serum level of 25-hydroxy vitamin D in the patients with alopecia areata. Methods： The infection of helieobacter pylori in the gastrointestinal tract was measured with 13Carbon Urea Breath Test （13C-UBT） in the patients with alopecia areata and detected the serum level of 25-OH vitamin D in the patients with alopeeia areata with the electrochemical luminescence assay respectively. Results： For the infection of the helicobacter pylori, no significant difference was found between the alopecia areata group and control group （P 〉 0.05）. Same situation was also found in the illness state, the course of disease and the disease severity within the alopeeia areata patients group （P〉0.05）. The detection of the vitamin D revealed that the serum level was distinctly lower in the patients with alopecia areata than that in the control group （P〈0.05）. The difference was also found between the different disease course （P〈0.05）. Conclusion： These results do not support the relation between helicobacter pylori and alopecia areata. The situations of the deficiency of 25-OH vitamin D is found in the patients with alopecia areata, which may play a role in pathological mechanism in this entity of alopecia.
