复发型原发性肾病综合征患儿肾组织P-gp170表达及临床意义

摘　　要：目的探讨复发型原发性肾病综合征(PNS)患儿肾组织P-糖蛋白170(P-gp170)的表达及临床意义。方法收集36例PNS患儿肾组织标本,包括对糖皮质激素敏感未治疗者(A组)7例,复发未用糖皮质激素治疗者(B组)8例,复发用糖皮质激素且未用环磷酰胺治疗1年者(C组)21例;另取10例正常肾脏标本作对照(D组)。免疫组织化学法检测各组肾组织P-gp170的表达;对P-gp170表达与首次复发和初发间隔时间、24-h尿蛋白定量、糖皮质激素用量作相关性分析。结果A、B、C组P-gp170表达高于D组(P<0.05),C组的表达高于A、B组(P<0.05)。B组P-gp170表达与首次复发和初发间隔时间呈负相关(r=-0.775,P<0.05),C组P-gp170表达与糖皮质激素用量呈正相关(r=0.798,P<0.05),A、B、C组P-gp170表达与24-h尿蛋白定量无明显相关(r分别为0.136、0.117、0.204,P均>0.05)。结论 P-gp170表达与糖皮质激素用量、PNS复发时间有关,可作为判断预后的指标。Objective To investigate the expression and clinical significance of P-glycoprotein 170（P-gp170） in children with repeated relapsed primary nephrotic syndrome （PNS）. Methods Thirty-six renal tissue biopsies of children with PNS were divided into three groups of A （steroidsensitive PNS without treatment, 7 cases）, B（relapsed PNS without glucocorticoids treatment, 8 cases） and C（relapsed PNS with glucocorticoids treatment but without cyclophosphamide, 21 cases）. Ten normal renal tissue biopsies were taken as the controls（group D）. Immunohistochemistry was used to detect P-gp170 expression in the renal tissue biopsies. The correlation between P-gpl70 expression and recurrence time, 24-h urine protein quantity or dosage of glucocorticoids was analyzed. Results P-gp170 expression was higher in groups of A,B and C than that in group D（P〈0. 05）,which was higher in group C than that in groups of A and B（P〈0. 05）. P-gp170 expression was negatively correlated with recurrence time in group B（r=-0. 775, P〈0. 05）, which was positively correlated with dosage of glueocorticoids in group C（r=0. 798,P〈0. 05）. No correlation was found between P-gp170 expression and 24-h urine protein quantity in groups of A, B and C（rl = 0. 136, r2 ： 0. 117, ra ： 0. 204, P〉0. 05）. Conclusion P-gpl70 expression is closely related with dosage of glucocortieoids and recurrence time, which mav be taken as an indicator for predicting the of children with PNS.
