Interlocking Intramedullary Nailing Combined with Single Cortical Screw Steel Plate for the Treatment of Humeral Nonunion

摘　　要：目的探讨交锁髓内钉联合单皮质螺钉钢板治疗肱骨骨不连的疗效。方法2004年10月～2011年4月，对8例肱骨中段骨不连患者应用交锁髓内钉联合单皮质螺钉钢板治疗，并行自体髂骨植骨。术后早期功能锻炼，定期拍片复查并门诊复查。结果植骨手术时间为70～150rain，出血量100～200ml；术后切口均一期愈合，无感染、内植物断裂等；术后患者肘关节活动优良率为100％，肩关节活动优良率为79％；4～8个月拍x线片均显示达到骨性愈合，平均5．1个月。结论交锁钉加单皮质螺钉钢板内固定治疗肱骨骨不连固定牢靠，可早期功能锻炼，并发症少，愈合率高。Objective To investigate the therapeutic effect of interlocking intramedullary nailing combined with single cortical screw steel plate on humeral nonunion. Methods 8 patients with humeral nonunion were treated with interlocking intramedullary nailing combined with single cortical screw steel plate from Oct. 2004 to Apr. 2011. Au- tologous iliac crest graft was conducted on all patients. Early function rehabilitation, X - ray examination and outpa- tient review were taken regularly after operation. Results It took 70 - 150 minutes for operation with the amount of bleeding 100 - 200 ml. All incisions were primary healing,no infection,hardware loosening or rupture was found. El- bow joint motion（ 100% ）as well as shoulder joint motion（79% ）were both good after operation. Bone union was ob- served by X - ray after 4 - 8 months （ 5.1 months in average）. Conclusion Interlocking intramedullary nailing com- bined with single cortical screw steel plate was a good option for humeral nonunion with early function rehabilitation, few complications and high healing rate.
