摘  要:目的:为了更合理使用CT增强对比剂,并为减少增强对比剂不良反应提供依据,本研究比较了CT增强对比剂的不同浓度和流速对肾脏增强图像质量的影响。方法:收集2012年1月至2012年6月在本院行腹部增强CT扫描的患者126例,所有病例均为无CT对比剂检查过敏者,并排除患有严重影响血液动力学分布的疾病。所有患者均行腹部CT平扫与增强扫描。依据增强扫描中CT对比剂的不同浓度和注射速率,126例患者随机分为3组:对比剂浓度300 mgI/mL,注射速率3 mL/s组;对比剂浓度350 mgI/mL,注射速率3 mL/s组;对比剂浓度350 mgI/mL,注射速率5 mL/s组。所有影像数据传至GE Adw 4.4工作站进行图像后处理与分析,于横断层面图像上分别选取左肾动脉开口处腹主动脉,左肾动脉,右肾动脉,左肾静脉,右肾静脉,左肾皮质,右肾皮质,左肾实质,右肾实质为感兴趣区,测量增强前后的CT值,根据测量值对上述结构影像强化情况进行观察,比对增强对比剂的不同浓度,不同注射速度在腹部影像增强图像中对肾脏影像质量的影响。结果:对比剂浓度300 mgI/mL,流速3 mL/s组腹主动脉、肾动脉、肾静脉、肾皮质、肾实质强化值(x±s)分别为287.74±49.83,276.24±44.57,141.61±20.87,136.67±24.50,168.31±25.45。对比剂浓度350 mgI/mL,流速3 mL/s组的以上结果分别为296.49±30.31,278.53±34.62,153.35±27.54,138.52±14.88,179.71±25.73。两组各指标差异无统计学意义(P>0.05)。对比剂浓度350 mgI/mL,流速3 mL/s组腹主动脉、肾动脉、肾静脉、肾皮质强化值(x±s)分别为296.49±30.31、278.53±34.62、153.35±27.54、138.52±14.88,对比剂浓度350 mgI/mL,流速5 mL/s组以上结果为411.83±54.13、405.69±54.03、207.98±37.21、199.32±55.33。两组以上指标差异有统计学意义(P<0.001)。结论:在肾脏增强扫描时,浓度不同低流速的两组对比剂的影像资料统计分析无显著性差异,建议单纯腹部脏器增强扫描时选�Objective:In order to use contrast agent reasonably and reduced incidence rate of contrast side-effect,the better contrast agent parameters in kidney CT contrast enhancement were established though compared the difference of agent concentration and flow rate.Methods:126 patients enrolled between January and June 2012 was accepted abdomen plain CT scan and contrast enhancement.Patients were excluded with allergy of contrast agent,severe hepatic cirrhosis,severe cardiac dysfunction,renal failure,severe hyperthyreosis,diabetes,abdominal occupying lesion and severe renal artery stenosis.126 patients were randomly allocated to receive different concentration and injection rate of CT agent(300 mgI/mL of agent concentration and 3 mL/s of injection rate;350 mgI/mL of agent concentration and 3 mL/s of flow rate;350mgI/mL of agent concentration and 5 mL/s of flow rate).All of the raw image data were exported to workstation(GE Adw 4.4) to analysis.CT values after injection of agents were measured in ROIs in abdominal aorta at the plane of left renal artery,left and right renal artery,left and right renal veins,left and right renal cortex and parenchyma at the transverse section,respectively.We observed the enhancement degree of that of tissue and evaluated the image quality in kidney with different kind of agent,different agent concentration and injection rate.Result:The enhancement values(x±s) of abdominal aorta,renal artery,renal vein,renal cortex,renal parenchyma from the group with the flow rate of 3 mL/s and the contrast concentration agent 300 mgI/mL were 287.74±49.83,276.24±44.57,141.61±20.87,136.67±24.50,168.31±25.45 respectively.The above index from the group with the flow rate of 3 mL/s and the contrast concentration agent 350 mgI/mL were 296.49±30.31,278.53±34.62,153.35±27.54,138.52±14.88,179.71±25.73 respectively.The test statistic analysis between two groups showed no significant difference(P>0.05);The enhancement values(x±s)of abdominal aorta,renal artery,renal vein,renal cortical from the group wi
