摘  要:目的 探讨新双源CT虚拟平扫 (virtual non-contrast CT, VNCT)代替常规平扫(true non-contrast CT, TNCT)在腹部应用的可行性研究。方法 对112例进展期胃癌患者行双源CT腹部常规平扫及动静脉期双能量扫描(100 kV/350 mA、Sn 140 kV/270 mA),经liver VNC软件处理得到动脉期虚拟平扫数据,比较常规平扫及虚拟平扫2组图像的图像质量及病灶检出率、平均CT值、信噪比(signal noise ratio,SNR)、对比噪声(contrast noise ratio, CNR)比及辐射剂量。结果 虚拟平扫图像质量有所下降(P〈0.05),但仍能满足诊断需求。进展期胃癌病灶检出率无差异;2组扫描图像所测器官平均CT值差异均有统计学意义(P〈0.05),虚拟平扫图像SNR及CNR明显高于常规平扫,差异有统计学意义(P〈0.05);取消常规平扫,辐射剂量可降低33%。结论 动脉期双能量虚拟平扫图像可以满足诊断需求,且降低了辐射剂量,SNR及CNR均高于常规平扫,具有潜在临床应用价值。Objective To investigate the feasibility of non-contrast CT(TNCT) being replaced by dual energy virtual non-contrast CT(VNCT) using dual-source CT in patients with gastric cancer. Methods 13.2 patients with progressive gastric cancer were per- formed abdominal CT scan. True non-contrast CT, arterial and portal phase enhanced scanning were performed by using dual-energy mode ( 100 kV/350mA and Snl40 kV/270 mA). The dedicated software of liver VNC was used to reformat virtual non-enhanced CT at the arterial phase. Image quality and lesion detectability, the average CT value, signal to noise ratio(SNR), contrast noise ratio (CNR) and radiation dose were compared. Results Although the image quality of VNCT was lwer than that of TNCT (P〈0.05), it still had moderate image quality to meet imaging diagnosis. There was no significant difference of the detection rate of advanced gastric cancer between VNCT and TNCT scan. The average CT value had statistical difference between TNCT and VNCT at the arterial phase (P〈0.05). SNR and CNR of VNCT were significantly higher than those of TNCT (P〈0.05). The radiation dose could be reduced by 33 ^ without the TNCT. Conclusion Image quality of VNCT can not only meet the needs of the diagnosis but also reduced patients' radiation dose. Compared with TNCT, VNCT has higher SNR, CNR and similar lesion detection rate, which make it has potential clinical role in abdomen deseases diagnosis.
