摘 要:目的 探讨对比剂用量及注射速率对多层螺旋CT脑动脉血管成像质量的影响。方法 采用GE 64层LightSpeed VCT对本院252例临床怀疑或确诊脑血管病患者行CT脑动脉血管造影。将患者随机分为A、B1、B2、C1、C2、D 6组,每组42例,对比剂用量及注射速率分别为A组70 mL、4.0 mL/s, B1组60 mL、4.0 mL/s,B2组60 mL、3.5 mL/s ,C1组50 mL、4.0 mL/s,C2组50 mL、3.5 mL/s,D组40 mL、4.0 mL/s;注射完造影剂后分别用20 mL、30 mL、30 mL、35 mL、35 mL、40 mL生理盐水以相应的速率冲刷。由3位有经验的高级技师双盲法对脑动脉成像质量进行评价。结果 A组与B1组、C1组、D组图像质量经统计学处理(等级资料的两样本秩和检验)P值分别为P〉0.5、0.20.5,差异无统计学意义。结论 64层螺旋CT脑动脉血管成像中,在保证图像质量的前提下可以适当减少对比剂用量及降低注射速率,以减少不良反应的发生率,降低对受检者的危害及减轻患者的经济负担。Objective To evaluate the influence of contrast agent amount and injection velocity on the image quality of multi-slice spiral CT cerebral artery angiography. Methods Cerebral CT angiography was performed in 252 patients with clinically suspected or confirmed cerebrovascular disease using GE 64-slice Light Speed VCT. Patients were divided in six groups based on different contrast agent amount , injection velocity and physiological saline amount as follow : group A (70 mL , 4.0 mL/s , 20 mL) ,group B1 (60 mL , 4.0 mL/s , 30 mL) , group B2(60 mL , 3.5 mL/s , 30 mL), group C1 (50 mL,4.0 mL/s,35 mL) ,group C2 (50 mL, 3.5 ml./s,35 mL), group D (40 mL,4.0 mL/s,40 mL}, each group involved 42 patients. The imaging quality of cerebral artery angiography was evaluated respectively by three experienced radiologists. Results The image quality had no significant difference between group A and group B1, group A and group C1 through two, sample Wilcoxon rank sum test of ranked data statistically,the P values were P〉0.5,0.2〈P〈0.4, respectively;there was significant difference between group A and group D, the P value was P 〈0. 001 ;between group B1 and group B2, group C1 and group C2 had no significant difference , the P values both were P〉0.5 . Conclusion The contrast agent amount and injection velocity can be reduced appropriately ensuring image quality at 64-slice CT cerebral artery angiography,and to reduce the incidence of adverse reactions, the harm to the subjects and to ease the economic burden of patients.
