ARB联用ACEI治疗老年糖尿病肾病的临床效果观察

摘　　要：目的:评价血管紧张素Ⅱ受体拮抗剂(ARB)和血管紧张素转换酶抑制剂(ACEI)联合用药治疗老年糖尿病的疗效。方法:80例临床确诊的糖尿病肾病(DN)患者按照入院顺序分为3组,ACEI组25例,ARB组25例,联合用药组30例,观察用药前、用药2个月后3组患者空腹血糖、血压、血肌酐、24h尿蛋白定量。结果:3组血压、24h尿蛋白定量均显著降低(P<0.05),联合用药组效果显著优于ACEI和ARB组(P<0.05)。结论:ACEI与ARB联合用药可提高糖尿病患者降压效果,降低蛋白尿。Objective： To investigate curative effects of ARB combined with ACEI on d senile dia- betic nephropathy. Methods. A total of 80 patients with senile diabetic nephropathy were randomly divided into ARB group （25 cases）, ACEI group （25 cases） and combined group （30 cases）. Patients in ARB group were treated with valsartan, patients in ACEI group were treated with benazepril, and patients in combined group were treated with valsartan and benazepril. Blood pressure, fasting blood-glucose, serum creatinine, 24 h urine protein before and 2 months after the treatment were measured and compared. Resuits： Blood pressure, 24 h quantitative urine protein were significantly lower in three groups, and the efficacy of the combined treatment group was significantly better than ACEI and ARB group（P〈0.05）. Conclusions： The combination of ACEI with ARB can improve blood pressure status and reduce uterine protein.
