摘  要:目的观察α-硫辛酸(ALA)联合甲钴胺治疗糖尿病周围神经病变(DPN)的临床疗效,并观察其对氧化应激的影响。方法将72例2型糖尿病并发DPN的患者随机分为观察组和对照组,每组36例。两组均给予甲钴胺治疗,观察组在此基础上加用ALA治疗,共4周。治疗前后,对两组患者行神经病变主觉症状问卷(TSS)评分,检查神经传导速度,检测血清丙二醛(MDA)水平和超氧化物歧化酶(SOD)水平。结果治疗4周后,与对照组比较,观察组TSS评分明显降低(P<0.01),神经传导速度明显加快(P<0.01),血清MDA含量明显降低(P<0.01),SOD活性则明显增加(P<0.01)。结论 ALA联合甲钴胺能显著改善DPN患者的临床症状,增加神经传导速度,减轻氧化应激。Objective To observe the effect of α-lipoic acid (ALA) combined with mecobalamin in the treatment of diabetic peripheral neuropathy (DPN) and its influence on the oxidative stress. Methods 72 patients with DPN were ran- domly divided into observation group and control group with each group of 36 cases, both group receiving intravenously mecobalamin, moreover, the observation group was given intravenously ALA for 4 weeks. The total symptoms scores (TSS), nerve conduction velocity, serum level of malonaldehyde (MDA) and superoxide dismutase (SOD) were com- pared before and after treatment in both groups. Results Compared with control group, the MDA was significantly de- creased (P 〈 0.01 ), SOD was significantly increased (P 〈 0.01 ), the nerve conduction velocity increased significantly (P 〈0.01 ), the TSS score dropped significantly (P 〈0.01 ) in observation group after treatment of 4 weeks. Conclusion ALA combined with mecobalamin can effectively improve the clinical symptoms, increase nerve conduction velocity and lighten the oxidative stress of the DPN patients.
