小儿肺炎支原体感染现状及其肺外损害相关实验室检测临床分析

摘　　要：目的了解肺炎支原体在儿童中的感染发病情况及其肺外损害的发生率,以期引起重视并得到早诊断、早治疗,尽量减少并发症及后遗症的发生。方法 2011年4月～2012年3月我院儿科住院部进行肺炎支原体检测患儿共1134例,均采用抽取静脉血或指尖末梢血行被动凝集法查肺炎支原体抗体IgM,阳性者同时行肝功、肾功、心肌酶谱、免疫、血沉、CRP等相关实验室检查。结果 1134例患儿中肺炎支原体抗体阳性感染率23.02%,>6～12岁段阳性比最高,为29.12%。冬春季发病率较高,阳性比分别占27.2%、32.95%。其中有203例作血生化检查,表现为肝功、肾功、心肌酶、免疫等有不同程度升高,以心肌酶CKMB升高者最多,达34.48%。结论肺炎支原体感染在儿童中发病率较高,以学龄儿多见,有向低年龄发病前移趋势。发病季节以冬春季为主。对肺外多脏器功能有损害,以心肌受损最多见。Objective To analysis the incidence of infection of mycoplasma pneumonia and extra-pulmonary leision in children and raise the emphasis of early diagnosis and reduce the happenness of the complication and sequela.Methods From April,2011 to march,2012,1134 patients from hospitalized pediatric department were recruited and detected with mycoplasmal pneumonia.They were all vein blood or capillary blood and use particle agglutination test was adopted for detection IgM of Mycoplasmal Pneumonia.Those who were positive were also test with liver function,Kidney function,myocardial enzymes,immunology,Erythrocyte Sedimentation Rate,CRP and other laboratory test.Results In the 1134 patients of Pneumonia,the positive rate was 23.02%,positive rate was 29.12% among age of above 6 to 12 years old.Winter and sping were among the higher incidence,represent 27.2% and 32.95% representatively.Among 203 cases were tested blood biochemical parameters,representing as increase of liver function,kidney function,immunology,and erythrocyte sedimentation rate,with highest percentage seen increase of myocardial enzymes,with 34.48%.Conclusion Mycoplasmal pneumonia infection is quite common in children,especially in School-age children and has a trend of younger and younger in children.more common in winter and spring.Harmful to the extra-pulmonary leision,especially common for heart tissue.
