摘　　要：目的研究房间隔缺损(ASD)介入治疗前后左心功能及血流动力学指标的变化。方法对45例ASD患者,术前经胸片、超声心动图(TTE)诊断。术前及术后3 d、1、6个月、1年经TTE检查左房前后径(LAD)、左室舒张末期前后径(LVEDD)、左室收缩末期前后径(LVESD)、主动脉内径(AOD)、肺动脉内径(PD)、肺动脉压力(PaSP)、每分输出量(CO)、每搏输出量(SV)、左室射血分数(EF)、左室短轴收缩率(FS)。结果 43例均成功置入封堵器,无主要手术相关并发症发生,所有患者封堵器位置、形态良好。LAD无明显变化,术后3 d LVEDD较术前明显增大(P<0.05)且术后1、6个月及1年较术后3 d增大(P<0.05),术后1个月LVESD较术前增大,术后6个月、1年继续增大(P<0.05),术后1年AD较术前增大(P<0.05);PaSP术后明显下降(P<0.05),术后CO、SV、EF、FS较术前明显增加(P<0.05)。结论介入治疗ASD手术操作简单,安全有效,患者术后左心功能及血流动力学指标改善明显。Objective To investigate the changes of left ventricle function and hemodynamics after transcatheterizition of atrial septal defects （ASD）. Methods 45 patients with ASD were diagnosed by physical examination, chest X -ray and transthoracic echo- cardiography （rITE）. TIE was implied to measure the left atrial diameter （LAD） , left ventricle end -diastole diameter （LVEDD） , left ventricle end - systole diameter （ LVESD ）, artery diameter （ AOD ）, pulmonary artery diameter （ PD ）, pulmonary artery pressure （PASP） , cardiac output （CO） , stroke volume （SV） , ejection fraction （EF） and fraction shortening （FS） at pre -operation and 3 - day, 1 - month, 6 - months and 1 - year poet - operation. Results 43 patients were implanted occluders successfully, there were not complications and all the oecluders located in proper position and the shapes were accurate in follow - up. LAD did not changed remark- ably, LVEDD, CO, SV, EF, FS increased significantly （ P 〈 0.05 ）. PASP decreased remarkably （ P 〈 0.05 ） , but AD increased sig- nificantly （ P 〈 0. 05） in fofiow - up. Conclusion Transcatheterizition of CHD - ASD is effective and safe. It can improve the left ven- tricle function and hemodynamics.
