急性低频下降型感音神经性听力损失的临床分析

摘　　要：目的探讨急性低频下降型感音神经性听力损失(ALHL)的临床特征和疗效,以指导该病的诊断和治疗。方法对ALHL患者的临床特征和临床听力学检查进行了回顾性分析,总结ALHL的发病特征。结果 32例ALHL(32耳)患者年龄26～48岁,男性10例(31.26%),女性22例(68.75%)。纯音听阈在125～500 Hz听力损失介于35～45 dB。22(22耳)例伴低调耳鸣,28例(28耳)伴耳闷胀感,9例(9耳)伴听觉过敏。32例患者(32耳)DPOAE低频引出率为16.2%,高频引出率为84.2%。所有患者ABR各波潜伏期及波间期正常。结论 ALHL以单耳发病为主,多见于中青年女性,常伴低调耳鸣、耳部胀满感。纯音听阈以低频听阈升高为特征,生活压力与本病发病可能相关,膜迷路积水、自身免疫和自主神经功能紊乱等因素的综合作用可能是导致本病的主要原因。Objective To study the clinical characteristics and treatment outcomes of acute low - tone sensorineural hearing loss （ALHL） patients, and to sum up experience of diagnosis and treatment. Methods Clinical characteristics and audiometric results of 32 patients with ALHL were retrospective analyzed between Jul. 2006 and Feb. 2010. Results 32 patients （32 ears） were 26 -48 years old, male 10 cases （31.26%）, female 22 cases （68.75%）. The pure tone average of 125 -500Hz was 35 -45dBHL. 22 ears compli- cated tinnitus. 28 ears complicated ear fullness and 9 ears complicated hyperakusis. The rate of DPOAE evoked was 16.2% in low - tone and 84.2% in high - tone. Conclusion ALHL appears to occur unilaterally, often complicates low - tone tinnitus and ear fullness. Audiometric test are characterized by low - frequency hearing loss. Its pathophysiology likely involves an array of abnormalities including endolymphatic hydrops, autoimmune disorders and autonomic nervous system dysfunction.
