平行双钢板内固定治疗肱骨干远端1/3关节外骨折14例  

Dual Plating Internal Fixation Techniques for Extra-articular Fractures of the Distal Third of Humeral Shaft: A Report of 14 Cases

摘　　要：目的探讨平行双钢板内固定治疗成人肱骨干远端1/3关节外骨折的临床疗效。方法对14例肱骨干远端1/3关节外骨折患者采用上臂下段后正中切口,纵行切开肱三头肌,直视下复位骨折,用平行双钢板内固定。结果14例患者切口均Ⅰ期愈合,无血管、神经损伤。14例均获随访,随访时间8～32个月,平均24个月。所有患者均骨折愈合,骨折愈合时间7～23周,平均11.5周。均未出现术后骨化性肌炎、关节粘连、骨折不愈合等并发症。肘关节功能按Morrey标准评定:优10例,良2例,中2例,优良率为85.7%。结论采用平行双钢板治疗成人肱骨远端1/3关节外骨折可以达到良好的复位和牢固的内固定,并早期行肘关节主动伸屈功能锻炼,未出现明显的并发症,骨折愈合良好。Objective To evaluate the clinical curative effect of the dual plating internal fixation techniques on extra-articular fractures of the distal third of humeral shaft.Methods Fourteen cases of extra-articular fractures of the distal third of humeral shaft underwent open reduction through posterior midline incision with triceps split approach under direct vision.The fractures were fixed with double plates and screws.Results All the 14 cases had primary healing without vascular and nerve damage.Patients were followed-up for an average of 24 months（range 8 to 32 months）.All fractures healed at an average of 11.5 weeks（range 7 to 23 weeks）.No complications such as myositis ossificans,joint adhesions and nonunion occurred after operation.According to Morrey evaluation standard,results were excellent in 10 of the 14 cases,good in 2 and fair in 2.The excellent and good rate was 85.7%.Conclusion The dual plating internal fixation techniques are effective for open reduction and internal fixation of extra-articular fractures of the distal third of humeral shaft and allow active flexion functional exercise without obvious complications.
