两种固定方法治疗桡骨远端不稳定骨折40例效果比较

摘　　要：目的探讨经切开复位解剖型钢板内固定术与有限内固定加外固定架固定术对桡骨远端不稳定骨折的疗效。方法选择资料完整的40例AO分类C型桡骨远端不稳定骨折,平均分为2组,分别采用切开复位解剖型钢板内固定术(解剖型钢板组)及有限内固定加外固定架固定术(有限内固定加外固定架组),对比研究两组疗效。结果①与解剖型钢板组比较,有限内固定加外固定架组手术时间短、术中出血量少,差异有统计学意义(P<0.05)。②复位质量:复位质量优者解剖型钢板组占35%,有限内固定加外固定架组占80%,两组比较差异有统计学意义(P<0.05)。③腕关节功能评定:功能评定优者解剖型钢板组占40%,有限内固定加外固定架组占75%,两组比较差异有统计学意义(P<0.05)。结论有限内固定加外固定架固定术具有创伤小、固定牢固等优点,很多方面优于切开复位解剖型钢板内固定术,推荐在AO分类C型桡骨远端骨折中使用。Objective To explore the effect of anatomical open reduction with steel sheet companion and limited internal fixed plus external frisket in treatment of unstable distal radius fracture.Methods Complete clinical data of forty cases of C Mode unstable distal radius fracture by AO classification were divided into anatomical open reduction plate internal fixation（group A,n = 20） and limited internal fixed plus external frisket fixation（group B,n = 20） and curative effect of two groups were compared.Results Operation time of group B was more shorter than that of group A,and bleeding volum of group B was also significantly less than that of group A（P 0.05）;reset quality： there was 35% cases of excellent reset quality in group A and 80% in group B,and the difference of the two groups was statistically significant（P 0.05）;function evaluation of wrist joint： there was 40% cases of excellent function evaluation in group A and 75% cases in group B,and the difference of the two groups was statistically significant（P 0.05）.Conclusion The method of limited internal fixed plus external frisket fixation has advantages such as little trauma and strong fixation,and many sides of this method are better than those of by the open reduction in anatomical plate internal fixation,which maybe more fit for C Mode unstable distal radius fracture by AO classification.
