摘  要:目的:观察骨蚀宁胶囊对激素性股骨头坏死患者凝血机制的影响。方法:将纳入研究的62例激素性股骨头坏死患者随机分为2组,骨蚀宁组32例,仙灵骨葆组30例。分别采用口服骨蚀宁胶囊和仙灵骨葆胶囊治疗。分别于治疗前和治疗12个月后抽血测定并比较2组患者的血浆血栓素B2、6-酮-前列腺素F1α、P选择素和血栓调节蛋白含量。结果:治疗前骨蚀宁组和仙灵骨葆组患者的血浆血栓素B2、P选择素、血栓调节蛋白和6-酮-前列腺素F1α含量比较,差异均无统计学意义[(27.95±7.08)ng.L-1,(28.99±4.43)ng.L-1,t=3.473,P=0.247;(11.76±2.11)μg.L-1,(11.43±1.26)μg.L-1,t=1.402,P=0.183;(7.15±1.74)μg.L-1,(7.84±0.91)μg.L-1,t=0.762,P=0.105;(52.35±13.61)ng.L-1,(44.27±7.54)ng.L-1,t=24.294,P=0.619]。治疗后2组患者血浆血栓素B2、P选择素和血栓调节蛋白的含量均下降[(16.19±4.88)ng.L-1,(20.90±4.09)ng.L-1;(6.40±1.81)μg.L-1,(7.65±1.01)μg.L-1;(3.55±1.28)μg.L-1,(6.21±1.62)μg.L-1],但骨蚀宁组下降更明显[(11.76±2.20)ng.L-1,(8.09±0.34)ng.L-1,t=6.824,P=0.000;(5.36±0.30)μg.L-1,(3.78±0.25)μg.L-1,t=4.215,P=0.000;(3.60±0.46)μg.L-1,(1.63±0.71)μg.L-1,t=4.162,P=0.000];治疗后骨蚀宁组和仙灵骨葆组患者的血浆6-酮-前列腺素F1α含量均上升[(161.04±19.46)ng.L-1,(119.80±14.21)ng.L-1],但骨蚀宁组上升更明显[(108.69±5.85)ng.L-1,(75.53±6.67)ng.L-1;t=39.203,P=0.000]。结论:骨蚀宁胶囊能缓解激素引起的血管内皮损害,抑制血小板聚集及预防血黏度增高,改善激素性股骨头坏死患者股骨头微循环和凝血功能,其作用优于仙灵骨葆胶囊。Objective:To observe the effect of GUSHINING capsule on blood coagulation mechanism for patients with steroid-induced necrosis of femoral head(SNFH).Methods:Sixty-two SNFH patients were included into the study and were randomly divided into 2 groups,32 patients were administrated with GUSHINING capsule(GUSHINING group),while the others were administrated with XIANLINGGUBAO capsule(XIANLINGGUBAO group).The contents of thromboxane B2(TXB2),6-keto-prostaglandin F1α(6-keto-PGF1α),P-selectin and thrombomodulin(TM)in blood plasma were detected and compared between the 2 groups before the treatment and 12 months after the treatment respectively.Results:There were no statistical differences in contents of TXB2,P-selectin,TM and 6-keto-PGF1α,in blood plasma between the 2 groups before the treatment[(27.95±7.08)ng·L-1,(28.99±4.43)ng·L-1,t=3.473,P=0.247;(11.76±2.11)μg·L-1,(11.43±1.26)μg·L-1,t=1.402,P=0.183;(7.15±1.74)μg·L-1,(7.84±0.91)μg·L-1,t=0.762,P=0.105;(52.35±13.61)ng·L-1,(44.27±7.54)ng·L-1,t=24.294,P=0.619].The contents of TXB2,P-selectin and TM in blood plasma of the 2 groups declined after the treatment[(16.19±4.88)ng·L-1,(20.90±4.09)ng·L-1;(6.40±1.81)μg·L-1,(7.65±1.01)μg·L-1;(3.55±1.28)μg·L-1,(6.21±1.62)μg·L-1],and there was a bigger decline in GUSHINING group compared to XIANLINGGUBAO group[(11.76±2.20)ng·L-1,(8.09±0.34)ng·L-1,t=6.824,P=0.000;(5.36±0.30)μg·L-1,(3.78±0.25)μg·L-1,t=4.215,P=0.000;(3.60±0.46)μg·L-1,(1.63±0.71)μg·L-1,t=4.162,P=0.000];while the plasma 6-keto-PGF1α contents for the 2 groups increased after the treatment[(161.04±19.46)ng·L-1,(119.80±14.21)ng·L-1],and there was a bigger increase in GUSHINING group compared to XIANLINGGUBAO group[(108.69±5.85)ng·L-1,(75.53±6.67)ng·L-1;t=39.203,P=0.000].Conclusion:GUSHINING capsule can improve microcirculation and blood coagulation functions of femoral head for SNFH pat
