多层螺旋CT扫描在诊断霉菌性副鼻窦炎中的应用价值（附20例分析）

摘　　要：目的探讨多层螺旋CT扫描诊断霉菌性副鼻窦炎的价值。材料：回顾性分析20例经病理证实的霉菌性鼻窦炎的多层螺旋CT影像表现。结果20例中单发17例，多发3例，上颌窦14例，累及筛窦4例，累及蝶窦2例。20例病变窦腔内见密度不均匀增高影，17例不均匀软组织块影可见高密度沙粒状、斑点状、条带状钙化影，12例窦腔周围骨质增生使骨质增厚致窦腔相对缩小，2例内侧壁骨破坏。结论CT在定性诊断方面具有特征性意义，以不均匀密度中夹杂高密度沙粒样、斑点状、带状钙化影为特征表现，与细菌性鼻窦炎及鼻窦肿瘤有鉴别价值。Objective： To study the feature of fungal sinusitis on the MSCT scanning. Methods ：CT findings were analyzed retrospectively at 20 fungal sinusitis cases confirmed by surgery and pathology. Results ：There were 17 cases of unilateral si- nusitis and 3 cases of double sinusitis. The lesions were often in maxillary sinus （ n = 14 ）, ethmoid sinus （ n = 4 ） and sphenoid sinus （ n =2 ） .The lesions were showed as heterogeneous density,with small calcified areas （ n=17 ） ; the local bone of proliferation cirrhosis made sinus dwindling （ n=12 ） and local bone destructions （ n=2 ）. Conclusion ： The mainly CT feature of fungal sinusitis was heterogeneous density of sinusal cavity with small calcification appeared as sand - like or patchy shadows. The CT was helpful to distingnish fungal sinusitis from chronic sinusitis
