螺旋CT对上尿路结石性梗阻并感染的诊断价值

摘　　要：目的探讨螺旋CT对上尿路结石性梗阻并感染的诊断价值。方法回顾分析30例临床诊断上尿路结石性梗阻并感染患者的CT图像。结果 30例(100%)CT均显示出上尿路结石性梗阻并不同程度积水,26例(86.7%)CT提示合并上尿路感染。CT提示感染的征象有:肾盂壁增厚、肾窦脂肪间隙模糊,肾实质增厚。密度减低,肾实质出现低密度灶,肾包膜毛糙,肾周脂肪间隙密度增高,肾筋膜增厚,肾周脓肿形成。结论螺旋CT对结石性上尿路梗阻并感染有重要的诊断价值;结石性上尿路梗阻合并感染时,肾脏和肾周可有炎症的CT表现,结合临床能做出准确诊断。Objective To study the diagnostic value of spiral CT in infection occurring in upper urinary tract stones. Methods The clinical data and the CT images of 30 patients with clinical diagnosis of infection occurring in upper urinary tract stones were retrospectively analyzed. Results The CT showed stones in upper urinary tract ob- struction and hydronephrosis in 30 cases （100%）. The CT prompted with infection occurring in upper urinary tract in 26 cases （86.6%）. The main CT characteristics of infection as foUows： renal pelvic wall thickening and the renal si-nus fat fuzzy, renal parenchyma swelling and lower density, Renal parenchyma appear low density lesion, Renal cap- sule rough, perirenal fat density increased, the renal fascia thickening, to the perinephric space abscess formation. Conclusion It has important value for Spiral CT to diagnostic the infection occurring in upper urinary tract obstruc-tion caused by stones. When infection occurring in upper urinary tract stones, there has inflammation CT findings in kidney and perinephric space, an accurate diagnosis can be able to make if combined with clinical.
