摘　　要：目的探讨自体游离股薄肌腱移植治疗KuwadeⅢ型陈旧性跟腱断裂的临床疗效。方法2009年6月至2011年12月，自体游离股薄肌腱移植治疗KuwadeⅢ型陈旧性跟腱断裂18例，术后给予石膏外固定，常规康复治疗，观察伤口愈合情况，测量小腿周径和小腿三头肌跖屈肌力，Amer-Lindholm标准评定踝关节的功能情况。结果所有18例患者均获得随访，平均随访时间15个月（7～20个月）。其中2例患者出现切口并发症，经换药后治愈，其余16例患者伤口一期愈合；所有18例患者患肢小腿的最大周径均小于对侧肢体，小腿三头肌肌力弱于对侧肢体，患肢踝关节的活动度与对侧相比，轻度受限，Amer-Lindholm标准评分6例为优，12例为良。结论自体游离股薄肌腱移植治疗陈旧性跟腱断裂的临床疗效确定，值得推广应用。Objective To evaluate the clinical efficacy of free gracilis tendon graft for the reconstruct of Ku-wade Ⅲ type chronic tears of the achilles tendon. Methods From June 2009 to December 2011, 18 patients with chronic achilles tendon rupture of Kuwade Ⅲ type were treated with free gracilis tendon. Plaster immobilization and routine rehabilitation therapy were performed postoperatively. The efficacy was evaluated by Amer-Lindholm criteri-on. Results All patients were followed-up for 7 to 20 months （mean 15 months）. Evaluated by Amer-lindholm crite-rion, the results were excellent in 6 cases, good in 12 cases and no worse case. Conclusion Free gracilis tendon was safe and perfect s to repair chronic Achilles tendon rupture of Kuwade Ⅲ type.
