摘 要:目的:研究sCD40L、IL-10在糖尿病及糖尿病微血管病变发生发展中的作用及其相关性。方法:选取2009年6月~2009年12月在我院内分泌科住院的2型糖尿病(DM2)患者(糖尿病无微血管病变组30例,糖尿病微血管病变组60例),健康对照组30例,采用ELISA测定患者血清sCD40L、IL-10(试剂盒购自上海西唐生物科技有限公司),并同时用血清采用酶学比色法测定血脂,采指尖血用糖化血红蛋白分析仪测定糖化血红蛋白(HbA1c)。结果:①DM2患者的HbA1c和TG高于正常对照组(P<0.05),DM2患者的TC、HDL、LDL和正常对照组无统计学差异(P>0.05),糖尿病微血管病变组的HbA1C、TC、TG、LDL、HDL和糖尿病无微血管病变组无统计学差异。②sCD40L糖尿病无微血管病变组高于正常对照组,糖尿病微血管病变组高于无微血管病变组。③IL-10水平糖尿病无微血管病变组低于正常对照组,糖尿病微血管病变组低于无微血管病变组。④sCD40L与IL-10呈负相关。结论:随着糖尿病病情的进展sCD40L水平逐渐升高,IL-10逐渐减低。Objective To study the correlativity between serum sCD40L, IL-10 and occurrence as well as development of diabet- ic microangiopathy. Methods The treatment of patients with type 2 diabetics who were hospitalized at endocrine division from June2009 to December 2009 was chosen as study object. The study included 30 cases for the groups of non- diabetic microangiopathy, 60 cases for the groups of diabetic microangiopathy and 30 cases for the groups of the healthy control, testing serum sCD40L and IL-10 by ELISA, while the serum was used for detemlination of blood lipid with zymology colorimetric method, and fingertip blood was collected to measure glycated hemoglobin by glycated hemoglobin analyzer. Results ( 1 ) There was no statistical difference (P 〉 0.05) in ages and sexes of the groups ; HbA1 c and TG of the type 2 diabetes were significantly higher than these in normal controls (P 〈 0.05), and there were no statistical difference in the TC, HDL, LDL of the type 2 diabetics compared with normal controls ( P 〉 0.05), and no statistical difference in HbAlc, TC, TG, LDL and HDL of diabetes mellitus with diabetic microangiopathy compared to the groups of non-diabetes microangiopathy. (2) sCD40L non-diabetes microangiopathy group was significantly higher than control group, diabetes micmangiopathy was obviously higher than group of diabetes without microangiopathy. (3) IL - 10 level of non-diabetes microangiopathy group was significantly lower than control group, diabetes microangiopathy showed prominently lower than group of diabetes without mi- cmangiopathy. (4) sCD40Lwas negatively correlated with IL-10. Conclusion Along with the progress of pathosis in diabetes, sCD40L level was rising and IL - 10 gradually declined
