摘 要:目的:综合观察及评价规律尿激酶封管法应用于血液透析患者永久性双腔导管中的使用效果。方法:将27例永久性双腔透析导管留置时间大于3个月的维持性血液透析病人随机分为尿激酶封管组、肝素封管组以及尿激酶肝素混合封管组,6个月后观察比较临床各项指标:透析血流量、导管通畅度以及三组单次抽吸导管不畅例次、动静脉反接发生例次及相关护理干预情况。结果:尿激酶封管组的导管通畅度、透析血流量与肝素组此较差异有显著意义(P<0.05);尿激酶封管组的导管通畅度、透析血流量与肝素尿激酶混合封管组比较差异无显著意义(P>0.05);尿激酶组因上机前发生单次抽吸导管不畅或动静脉反接事件时需护理干预例次较肝素组明显减少,较混合组无明显差异。结论:规律性使用尿激酶封管可有效用于永久性双腔透析导管留置患者。Objective: To explore the effect of urokinase in tube sealing for permanent cuffed catheter in hemodialysis patients. Methods: 27 hemodialysis patients were randomly divided into 3 groups: urokinase group (n=9), heparin group(n=9) and urokinase+heparin group (n=9). All kinds of clinical indexs were observed and compare with different groups after 6 months. Results: Compared with heparin group, case numbers of catheter unobstructed degree and dialysis blood flow were signigicantly increased (P 〈0.05) ; numbers of single suction catheter impeded, arteriovenous reverse connect and nursing intervention were significantly reduced (P〈0.05). Moreover, there were no markedly differences as compared to urokinase+heparin group (P〉0.05). Conclusion: Regular use of urokinase sealing tube can be effectively used for permanent cuffed catheter in hemodialysis patients.
