补肾祛斑汤治疗黄褐斑的临床研究

摘　　要：目的:探讨补肾祛斑汤治疗黄褐斑的临床疗效及其部分作用机制。方法:将275例辨证为肾虚型的黄褐斑患者按随机数字表法分为补肾祛斑汤治疗组(简称治疗组,139例)和维生素C、E对照组(简称对照组,136例)。分别予以30d药物治疗,3个月后评定疗效,并检测两组血清雌二醇(E2)、睾酮(T)、孕酮(P)、超氧化物歧化酶(SOD)、过氧化脂质分解产物丙二醛(MDA)及血液流变学水平变化。结果:治疗组总有效率为85.61%,对照组总有效率为50.73%,两组比较差异有统计学意义(P<0.05)。治疗组的血液流变学各指标在治疗后明显下降,且治疗组与对照组相比,有显著性差异(P<0.05);治疗组治疗后MDA较治疗前明显降低(P<0.05),SOD明显升高(P<0.05),且治疗组与对照组相比,有显著性差异(P<0.05);治疗组治疗后E2升高(P<0.05),T降低(P<0.05),但仍在正常范围内,与对照组相比,有显著性差异(P<0.05),两组P水平变化不明显。结论:补肾祛斑汤对于黄褐斑的疗效满意,其机制可能与改善患者氧化与抗氧化失衡,调节患者性激素水平,改善患者血液流变学有关。Objective： To explore the clinical efficacy of Bushen Quban Decoction on melasma.Methods： 275 patients were randomly divided into 2 groups.The treatment group were given Bushen Quban Decoction,while the control group were given with western medicine（Vit E and Vit C）.30 days later,the clinical effect were compared.And the level of estradiol（E2）,testosterone（T）,progesterone（P）,superoxide dismutase（SOD）,malondialdehyde（MDA） and the hemorheology were obtained to evaluate the efficacy of the two treatments.Results： After 30 days’ treatment,the toal effective rate was higher in treatment group（85.61%） than in control group one（50.73%）.The clinic effect between the treatment group and control group had significant difference（P﹤0.05）.In treatment group,the levels of MDA after treatment were lower as compared with those before treatment（P0.01）,while the level of SOD was elevated,compared with pre-treatment there is statistically significant（P﹤0.01）.Furthermore,there was significantly different in comparison of two groups（P﹤0.05）.After treatment,the level of E2 was elevated and the level of T had a remarkably fall（P﹤0.01）,while still in the normal range.There was significantly different in comparison of two groups（P﹤0.05）.In two groups,there were no obvious changes in P before and after treatment.Conclusion： Bushen Quban Decoction has reliable effects on melasmat,the mechanism of which might be related to its effects in regulating sex hormone on levels,improving the hemorheology and antioxidize function of patients.
