彩色多普勒超声在儿童神经母细胞瘤诊断和随访中的价值

摘　　要：目的:评价彩色多普勒超声(color doppler ultrasound,CDFI)在儿童腹部神经母细胞瘤诊断和随访中的价值。方法:回顾性分析经手术及病理证实的26例儿童神经母细胞瘤的彩色多普勒图像。结果:26例患儿中,肿瘤原发于肾上腺20例(左12,右8),来源于椎旁交感神经节6例,肿瘤超越腹正中线者8例,肿瘤包绕大血管、周围脏器和血管推挤受压6例,侵犯肾脏2例,肝转移1例,腹膜后淋巴结转移9例。21例术前做出正确诊断,术前超声诊断符合率80.8%,术后证实的23例肿瘤内钙化中,超声检出20例,检出率87.0%。20例行1-4次随访复查。结论:超声对儿童神经母细胞瘤诊断和随访有重要价值。Objective：To evaluate the value of color doppler ultrasound （CDFI） in diagnosis and follow - up of pediatric neuroblastoma. Methods： Retrospectively analyze color doppler images of 26 cases of children neuroblastoma confirmed by surgery and pathology. Results： Among the 26 cases, primary adrenal tumors in 20 patients （ left 12, fight 8 ） , tumor derived from the paravertebral sympathetic ganglia in 6 cases, tumor beyond the ventral midline in 8 cases, tumor wrapped around large vessels, organs or tumor compressed vessels in 6 cases,invasion of the kidney in 2 cases, liver metastasis in 1, retroperitoneal lymph node metastasis in 9 cases. The accurate diagnosis was got in 21 patients before operation. The accurate diagnosis rate of ultrasound was 80. 8% , postoperative confirmed 23cases within a tumor calcification~ Ultrasonic detection for 20 cases, the detection rate was 87%. 20 patients were followed up 1 to 4 times by ultrasound. Conclusion ： Ultrasound was very important for diagnosis and follow - up of pediatric neuroblastoma.
