摘 要:目的应用超声测量肝前脂肪最大厚度(Vmax),预测其在冠状动脉疾病的发生及严重性。方法选自我院行冠状动脉造影的患者185例,根据有无冠状动脉狭窄进行分组,其中冠心病患者144例,非冠心病患者41例。应用超声测量两组患者的Vmax及皮下脂肪厚度最小值(Smin),并对两组各参数进行比较分析。通过绘制受试者特征曲线比较分析Vmax与传统预测冠状动脉疾病的危险因素腰围预测冠状动脉疾病的能力。结果冠心病组患者Vmax增加,与非冠心病组比较差异有统计学意义(P<0.001),Smix无明显增加;Vmax随病变血管数目增多而增加(P<0.05),Vmax与Smin呈低度正相关(r=0.397,P<0.001)。ROC曲线分析Vmax预测冠状动脉疾病的临界值分别为0.84cm,敏感性73.6%,特异性68.3%,曲线下面积为0.716,P<0.001,较腰围(敏感性72.9%,特异性58.5%,曲线下面积0.612,P=0.028)能更好地预测冠状动脉疾病。结论 Vmax能独立预测冠状动脉疾病发生和严重性,与传统预测冠状动脉疾病的危险因素腰围比较,超声测量Vmax可为临床提供新信息。Objective To investigate the value of abdominal visceral fat maximal thickness (Vmax) measured by ultrasonography in predicting the presence and severity of coronary artery disease (CAD). Methods One hundred and eighty- five patients who were hospitalized to undergo coronary angiography entered the study, the patients were divided into CAD group (144 patients) and no CAD group (41 patients) according to the degree of coronary artery stenosis. The Vmax and minimum subcutaneous fat thickness(Smin)were measured by ulrtasonography. The parameters of the two groups were eomparely analyzed. Results Compared with 41 patients without coronary artery disease,Vmax was increased in CAD group, there was significant difference (P〈0.001). Stain was not increased obviously. Vmax was increased with the increased number of lesioned blood vessels (P〈0.05).Vmax was weakly positively correlated with Smin (r=0.397,P〈0.001). ROC showed the critical value of Vmax in predicting CAD was 0.84cm, the sensitivity was 73.6%, the specificity was 68.3%, the area under the curve was 0.716, P〈0.001. Vmax could better predict CAD than waist circumference (the sensitivity of 72.9%, the specificity of 58.5%, the area under the curve of 0.612,P=0.028). Conclusion Vmax can independently predict the presence and severity of CAD, compared with traditional risk factors, Vmax measured with ultrasound can provide new information for clinic.
