冈上肌出口位摄影方法的改良及临床应用

摘　　要：目的:探讨冈上肌出口位的摄影方法及技巧,及其在临床的应用价值。方法:对常规冈上肌出口位摄影方法进行改良、简化,采取前后位水平投照的方法,对92例因肩关节疼痛或外伤的患者拍摄冈上肌出口位片,并加以分析。结果:肩胛骨侧位投影成一明显"Y"字,肩峰形态、骨质结构显示清晰;肩峰分为三型:扁平型(Ⅰ型),弯曲型(Ⅱ型)和钩型(Ⅲ型)。80例非外伤患者中,Ⅰ型21例(26%),Ⅱ型28例(35%),Ⅲ型31例(39%);12例外伤患者中8例可见骨折或脱位征象。结论:对冈上肌出口位摄影方法的改良、简化是可行的,能达到临床诊断的目的;冈上肌出口位摄片对肩峰下撞击综合征的诊断具有重要参考价值,对外伤所致的肩关节骨折或脱位的诊断也有临床应用价值。@@@@Objective:To explore the projection method and technique for supraspinatus outlets and their value for clinical applications.Method:The conventional projection method for supraspinatus outlets was modified and simplified;films of 92 patients of shoulder joint pains or traumas were taken with the method of front and rear horizontal projection,and were then analyzed.Result:Lateral projections of scapulae clearly exhibited a Y shape,and the acromion form and bone structure were clearly manifested.There were three type of acromion:flat(TypeⅠ),bent(TypeⅡ),and hook-shaped(TypeⅢ).Of the 80 non-trauma patients,21 were TypeⅠ(26%),28 were TypeⅡ(35%),and 31 were TypeⅢ(39%).Of the 12 trauma patients,eight had fracture or dislocation symptoms.Conclusion:The modification and simplification of the projection method for supraspinatus outlets are feasible and able to achieve the goal of clinical diagnosis.The supraspinatus oulet films have significant reference value in the diagnosis of the Subacromial Impingement Syndrome;they also have clinical application value in the diagnosis of trauma-caused fractures or dislocation of shoulder joints.
