摘　　要：目的:探讨早产儿羊水、脐血中IL-6、IL-8、TNF-α水平变化与支气管肺发育不良(BPD)的关系。方法:留取2010年1月～2012年6月分娩胎龄<34周的早产儿的羊水、脐血及胎膜,根据胎膜病理是否有绒毛膜羊膜炎(CAM),分为CAM组及非CAM组,检测羊水、脐血相关细胞因子,随访BPD发生率,回顾性分析相关细胞因子水平与BPD相关性。结果:CAM组的BPD发生率及胎膜早破发生率、早破时间与非CAM组比较,差异有统计学意义(P<0.05);CAM组BPD发生率明显高于非CAM组(P<0.05),CAM组羊水、脐血IL-6、IL-8水平明显高于非CAM组(P<0.05)。结论:检测羊水或脐血中IL-6、IL-8水平,对于BPD的早发现、早干预,降低严重并发症的发生率,有积极的临床意义。Objective：To explore the relationship between interleukin-6（IL-6）,interleukin-8（IL-8）,and tumor necrosis factor-α（TNF-α） levels in amniotic fluid and umbilical cord blood of premature infants and bronchopulmonary dysplasia（BPD）.Methods：The samples of amniotic fluid,umbilical cord blood,and fetal membrane of premature infants less than 34 gestational weeks who were born from January 2010 to June 2012 were collected,then the premature infants were divided into chorioamnionitis（CAM） group and non-chorioamnionitis（non-CAM） group according to pathological examination of fetal membrane;the related cytokines in amniotic fluid and umbilical cord blood were detected;the incidence rate of BPD was followed up;the correlation between the levels of related cytokines and BPD was analyzed retrospectively.Results：There were statistically significant difference in the incidence rates of BPD and premature rupture of membrane（PROM）,and the time of PROM between CAM group and non-CAM group（P0.05）;the incidence rate of BPD in CAM group was statistically significantly higher than that in non-CAM group（P0.05）,the levels of IL-6 and IL-8 in amniotic fluid and umbilical cord blood of CAM group were statistically significantly higher than those in non-CAM group（P0.05）.Conclusion：Detecting the levels of IL-6 and IL-8 in amniotic fluid or umbilical cord blood has active clinical significance for early detection,early intervention of BPD and reducing the incidence rates of severe complications.
