摘　　要：目的对592例呼吸道感染住院儿童进行呼吸道合胞病毒(HRSV)感染监测及亚型鉴定,探讨其流行规律。方法于2010年3月至2011年2月间采集四川大学妇女儿童医院因呼吸道感染住院儿童的鼻咽抽吸物,使用RT-PCR方法检测HRSV及鉴定亚型,收集病历资料进行统计分析。结果 592份呼吸道鼻咽抽吸物检出HRSV感染145份(24.5%),其中A亚型107份(73.8%),B亚型38份(26.2%)。HRSV感染高峰季节在冬季,患儿年龄多为2岁以内,临床诊断以肺炎和支气管炎为主;多因素非条件Logistic回归分析表明年龄、季节和潜在基础性疾病对病毒感染有影响,年龄2岁以下、感染季节冬季和潜在基础性疾病是HRSV感染的危险因素。结论 HRSV是2010～2011年四川婴幼儿呼吸道感染的主要病原之一,A亚型毒株在此流行期占优势地位。四川HRSV感染监测结果为全国HRSV流行病学研究积累了资料。OBJECTIVE To understand epidemiological features for 592 cases of human respiratory syncytial virus （HRSV） infection in hospitalized children with respiratory tract infection in Sichuan. METHODS Nasopharyngeal aspirates collected from 592 hospitalized children were detected and sub-typed for HRSV using RT-PCR from March 2010 to February 2011. Case information was also gathered for statistical analysis. RESULTS Out of 592 specimens, 145 （24.5%） were positive for HRSV, subtype A and B accounted for 107 （73.8%） and 28 （26.2%）, respectively. After comprehensive analysis of case information, the high peak season of HRSV infection was in winter in Sichuan, the majority of infected children was under 2 years old, and the clinical diagnosis was dominated by pneumonia and bronchitis. Logistic regression analysis demonstrated that factors influencing HRSV infection incidence were age, season and underlying disease. CONCLUSION HRSV has been one of the major causes of respiratory tract infection in children in Sichuan area since 2010 to 2011, and subtype A strain is predominant during this period. The monitoring results are useful to the epidemiologic study of HRSV of the whole nation.
