关节镜手术诊治早、中期单纯膝关节结核的临床体会

摘　　要：目的探讨关节镜诊治单纯早、中期膝关节结核的临床疗效。方法回顾性地分析该院2011年3例进行关节镜手术探查及治疗并最终确诊为膝关节结核的病例,分析术前、术中、术后诊断符合情况;探讨经关节镜手术探查及治疗后短期内(术后3个月)患者的疗效。结果 3例单纯早中期膝关节结核患者因入院前无结核病史及典型结核症状,术前均未明确诊断为膝关节结核。经关节镜手术探查在术中诊断为膝关节结核可能,术后经膝关节坏死组织病理报告及行相关结核抗体检查确诊为膝关节结核;经关节镜探查及治疗后,短期内(术后3个月)患者膝关节疼痛减轻,膝关节功能较术前改善。结论如无结核病史及典型结核症状,单纯早中期膝关节结核在术前、术中不易诊断,需坏死组织病理检查才能明确诊断为膝关节结核,经关节镜手术治疗患者创伤小,在短期内有一定疗效。【Objective】To study the clinical efficacy of arthroscopic surgery for treatment of early and mid purely knee tuberculosis.【Method】Three cases finally diagnosed as knee joint tuberculosis with arthroscopic surgical exploration and treatment in our hospital in 2011 were retrospectively analyzed during preoperative, intraoperative, postoperative diagnosis.【Results】Because of no history of tuberculosis and typical symptoms of TB, three cases were not diagnosed as knee tuberculosis before admission. Arthroscopic surgical exploration for the knee joint tuberculosis may be diagnosed during surgery, they were diagnosed as knee tuberculosis with postoperative knee necrotic tissue pathology report and line-related tuberculosis antibody report; three cases had pain relief after arthroscopic exploration and treatment in the short term and improved knee function than before.【Conclusion】The diagnosis of early and mid purely knee tuberculosis is difficult in the preoperative and intraoperative if there is no history of tuberculosis and typical symptoms of TB. To confirm the diagnosis need the knee necrotic tissue pathological examination. Arthroscopic surgery for knee tuberculosis is certain effective in the short term and minimally invasive.
