利用合理用药监测系统对内科临床住院患者用药医嘱的监测分析

摘　　要：目的利用合理用药监测系统(PASS)对内科住院患者医嘱进行监测,促进药物的合理应用。方法利用嵌套了PASS的"创业医院资源管理平台(BSHRP)"的临床药学工作站,回顾监测我院2012年第1季度内科住院部6个科室1 412位患者的医嘱,并对监测结果进行分析。结果 PASS提示用药问题医嘱4 440次,用药问题医嘱发生率为0.58次/每病人住院天。其中药物不良相互作用发生率0.58次/每病人住院天,注射液体外不合理配伍发生率0.007次/每病人住院天,儿童用药警告发生率0.000 4次/每病人住院天,给药途径监测发生率0.000 8次/每病人住院天。结论 PASS系统审查医嘱方便、快捷,有力促进了药物的合理应用;PASS提示的用药问题医嘱不完全等于不合理用药,需进一步分析后应用,PASS系统的功能尚需进一步完善。Objective To analyze the monitoring on inpatient medical orders in our hospital by Prescription Automatic Screening System（PASS）,and to promote the rational drug use.Methods 1 412 patients′ medical orders from January to March in 2012 were screened by PASS equipped in ＂B-Soft Hospital Resource Planning（BSHRP）＂ retrospectively.Results There were 4 440 irrational drug use medical orders,and the incidences of irrational drug use stood at 0.58 time/person·day,of which 0.58 time/person·day involved drug adverse interactions,0.007 time/person·day involved injection combinations,0.000 4 time/person·day involved in irrational use in children and 0.000 8 time/person·day involved in irrational use in usage.Conclusion The monitoring in medical orders is convenient and effective by PASS and it can help to promote the rational drug use.The PASS monitoring results don′t mean those medical orders are wrong,so we still need to use them for further analysis.PASS still need to be updated function to meet the clinical pharmacy needs.
