血清Chemerin水平在糖尿病肾病中的变化

摘　　要：目的观察不同阶段糖尿病肾病(DN)患者血清中chemerin的水平变化,探讨其与DN的相关性。方法 2型糖尿病(T2DM)患者104例分为正常蛋白尿组(NUAlb组)、微量蛋白尿组(MUAlb组)及大量蛋白尿组(CUAlb组),对照组(NC组)为28名健康体检者。应用酶联免疫分析法(ELISA)测定血清chemerin。结果 CUAlb组血清chemerin水平较NC组、NUAlb组及MUAlb组明显升高(P﹤0.01),chemerin水平与病程、BMI、SBP、TG、FPG、HOMA-IR、HbA1c、BUN、Cr均呈正相关(P﹤0.01),与Ccr、HDL-C呈负相关(P﹤0.01)。线性回归分析示SBP、BUN、Ccr及HDL-C与血清chemerin水平关系密切。结论 2型糖尿病肾病患者血清chemerin水平升高可能预示肾功能减退及糖尿病微血管病变的进展。OBJECTIVE The association of serum chemerin levels with type 2 diabetic nephropathy （DN） was unkown, the aim of this study was to investigate the relationship between serum chemefin and type 2 diabetic nephropathy. METHODS A total of 104 type 2 diabetic patients with normoalbuminufia （NUAlb group）, microalbuminuria （MUAlb group） and macroalbuminutia （CUAlb group）, and 28 healthy controls （NC group） were enrolled evenly.The serum chemerin levels were determined by enzyme-linked immuno sorbent assay （ELISA）. RESULTS The serum chemerin levels was significantly elevated in CUAlb group compared with the other groups （P 〈 0.01 ）. The serum chemerin levels was positively correlated with the course of disease, BMI, SBP, TG, FPG, HOMA-IR, HbAle, BUN, Cr （P〈0.01）, and negatively correlated with Ccr, HDL-C （P〈 0.01 ）. Linear regression analysis showed that SBP, BUN, Ccr and HDL-C remained significantly associated with the serum chemerin. CONCLUSION The high level of chemerin in DN may indicate the development of renal insufficiency and the lesions of microangiopathy.
