摘  要:目的探讨手术患者记忆障碍的患病情况及术中失血量对记忆功能的影响。方法选择符合标准的外科手术患者282例,于术前24h、术后24h采用行为记忆测验第2版(RBMT-Ⅱ)进行记忆功能的测评,并采用称重法、计量法、目测法统计术中出血量。结果 282例患者中术后记忆障碍发生率为68.8%,其中轻度记忆障碍占34.8%,中度记忆障碍占33.0%,重度记忆障碍占1.1%;术中失血量越多的患者术后记忆障碍发生率高,且程度重,差异有统计学意义(P﹤0.01或P﹤0.05)。结论术中失血量对术后患者的记忆功能有一定的影响,因此,采取术中干预措施,减少术中失血量,对降低术后患者记忆障碍的发生率,延长患者生存时间,提高患者的生活质量具有重要的意义。OBJECTIVE To explore the incidence and the influence of blood loss during operation on postoperative patients with memory disorders. METHODS 282 patients were assessed the memory functions using Rivermead Behavior Memory Test II (RBMT-II ) in the 24 hours before and after operation and the amount of blood loss during operation were measured by weighing method, measurement method and experience estimate method. RESULTS It was showed that among the 282 postoperative patients, the prevalence of memory dysfunction was 68.8%, of which mild memory dysfunction was 34.8% ; moderate memory dysfunction was 33.0%; severe memory dysfunction was 1.1%. The more per-operative bleeding lossed, the higher incidence and heavier degree of memory disorder in postoperative patients was, with statistical difference (P 〈 0.01 or P 〈 0.05). CONCLUSION Blood loss during operation has an impact on memory functioninpost-operative patients, therefore, taking measures to reduce intraoperative blood loss is very important to reduce the incidence of postoperative patients memory disorders, to extend survival time and to improve the quality of life of the patients.
