动态监测血清超敏C反应蛋白对老年吸入性肺炎患者的临床意义

摘　　要：目的探讨动态监测老年吸入性肺炎患者血清超敏C反应蛋白(hs-CRP)水平在本病诊治中的临床意义。方法选择48例老年吸入性肺炎患者,检测其确诊当日、第3天和第7天血清hs-CRP水平,并结合患者的病情变化进行分析和比较。结果本组48例老年吸入性肺炎患者经积极治疗后,治愈或明显缓解36例,死亡12例,死亡率为25%;患者确诊当日、第3、7天血清hs-CRP水平分别为(20.37±6.51)mg/L、(87.35±20.18)mg/L和(33.47±19.85)mg/L,其不同时间点差异具有统计学意义(P<0.01);第3天和第7天患者血清hs-CRP水平在病情危重组均明显高于病情较轻组,差异具有统计学意义(P<0.01)。结论老年吸入性肺炎患者血清hs-CRP水平动态变化与病情进展具有相关性,动态监测血清hs-CRP水平可有助于老年吸入性肺炎患者病情判断及指导治疗。Objective To study the value of high-sensitivity C-reactive protein （hs-CRP） in the treatment of the elderly patients with aspiration pneumonia. Methods A total of 48 elderly patients with aspiration pneumonia were selected. The levels of hs-CRP were detected on the day of diagnosis （1st day） ,the 3rd day and the 7th day, which were analyzed and compared in combination with the changes in patient＇ s conditions. Results After the treatment, in 48 elderly patients with aspiration pneumonia, 36 were cured or obtained relief, and 12 died with a mortality rate of 25%. On the lst,3rd, and the 7th day,the hs-CRP levels were（20.37 ~ 6.51 ） , （87.35 _＋20.18） and （33.47 ＋- 19.85） mg/L, respectively, and there were significant differences among the different time points （P 〈0.01 ）. On the 3rd and the 7th day, the hs-CRP levels in the severe group were significantly higher than that in the mild group （ P 〈 0.01 ）. Conclusion Hs-CRP level is correlated with changes in condition in elderly patients with aspiration pneumonia. Dynamic observation of Hs-CRP level can effectively reflect the disease severity and progress.
