摘 要:目的研究选择性脱细胞羊皮生物敷料促进大鼠深Ⅱ度烧伤创面愈合及对大鼠T细胞亚群的影响。方法制备选择性脱细胞羊皮生物敷料,将其制成浸提液培养基培养成纤维细胞,检测其细胞毒性。成年健康Wistar大鼠72只,随机分为三组:生物敷料治疗组(选择性脱细胞羊皮生物敷料覆盖创面)和碘伏纱布组(碘伏纱布覆盖治疗创面)各32只大鼠,正常对照组8只大鼠。在术后3d、7d、14d和21d四个时间段检测生物敷料治疗组和碘伏纱布组的创面愈合率,并运用流式细胞仪检测术后大鼠外周血中CD3+、CD4+、CD8+和CD4+/CD8+表达变化情况并进行比较,同时与正常对照组数据进行比较。结果选择性脱细胞羊皮生物敷料对成纤维细胞增殖没有明显的影响;生物敷料治疗组大鼠深Ⅱ度烧伤术后14d和21d时,创面愈合明显,创面缩小,创面愈合率较碘伏纱布组有明显的提高(P<0.05)。术后生物敷料治疗组术后3dCD4+/CD8+比值(0.95±0.15)较正常大鼠CD4+/CD8+比值(1.87±0.28)下降明显,随着治疗时间的持续,生物敷料治疗组CD4+/CD8+的值逐渐升高,在术后21d时CD4+/CD8+的值(1.82±0.26)接近正常对照组的值。生物敷料治疗组术后14dCD4+/CD8+比值(1.59±0.23)与碘伏纱布组术后14d(1.37±0.17)比较有一定的提高,差异有统计学意义(P<0.05)。结论选择性脱细胞羊皮生物敷料对细胞生长无明显毒副作用;选择性脱细胞羊皮生物敷料促进烧伤大鼠免疫系统尽早恢复正常平衡,并加速深Ⅱ度烧伤创面愈合。Objective To study the effects of selective sheep acellular dermis matrix on rats with deep partial thickness burn and T cell subsets influences. Methods Prepared selective sheep acellular dermal matrix leaching liquid culture medium of cultured fibroblasts, detected their cytotoxicity. Used healthy adult Wistar rats to make deep burns model, then randomly divided them into two groups: selective sheep acellular dermal matrix treatment group and iodophor gauze treatment group. Detected the rate of wound healing in postoperative 3,7,14 and 21 d, and used flow cytometry to detect rat peripheral CD3+ , CD4+ , CD8+ and CD4 +/ CD8+expression. Results Selective sheep acellular dermal matrix treatmenthad no significant effect on the proliferation of fibroblasts; selective sheep acellular dermal matrix treatment of burn rat at 14 d and 21 d, showed wound healing significantly, the wound healing rate being higher than iodophor gauze treatment group ( P 〈 0.05 ). In the selective sheep acellular dermal matrix treatment group 3 d, prostoperation the CIM + / CD8+ ratio (0.95± 0.15) significantly decreased compared with normal rat CIM +/CD8+ ( 1.87 ±0.28) , but gradually increased 21 d postoperation ( 1.82±0.26), similar to normal rats. In the selective sheep acellular dermal matrix treatment 14 d group 14 d postoperation CIM +/ CD8+ ratio ( 1.59± 0.23) increased compared with the povidone-iodine gauze treatment group ( 1.37 ± 0.17) (P 〈 0.05). Conclusions Selective sheep acellular dermal matrix treatment had to apparent side effects on cell growth; selective sheep acellular dermal matrix treatment promoted burned rat immune system to restore to normal balance and accelerate deep degree burn wound healing as early as possible.
