摘  要:目的:观察慢性阻塞性肺疾病(COPD)大鼠尿半胱氨酸蛋白酶抑制剂-C(Cys-C)mRNA表达水平的变化,探讨COPD对肾脏功能的影响。方法:将大鼠随机分为空白组(N)、COPD组(B)、COPD加肾损害组(D)、单纯肾损害组(E),每组8只,用改良烟熏加气管滴脂多糖(LPS)法建立COPD模型,腺嘌呤灌胃制作肾损害模型,RT-PCR法检测大鼠尿Cys-C mRNA表达,ELISA法检测肺组织匀浆基质金属蛋白酶-9(MMP-9)含量。结果:与空白组相比,各实验组Cys-C mRNA的表达均显著增加(P<0.01),COPD加肾损害组mRNA的表达比稳定期组升高(P<0.05),单纯肾损害组表达比稳定期升高(P<0.05);与空白组相比,各实验组肺组织匀浆中MMP-9的含量均显著增高(P<0.01),COPD加肾损害组比稳定期组含量升高(P<0.05)。结论:在COPD中,肺脏功能的损伤对肾脏功能有一定的影响,COPD可引起一定的肾损害,提示在COPD病程中肺肾之间的相关性,及COPD治疗过程中应注意对肾脏的保护。Objective:To observe the expression levels of the urine cystatin-C(Cys-C) mRNA in the chronic obstructive pulmonary disease(COPD) rats,and explore the impact of COPD on the kidney function.Methods:Divide the rat into three groups randomly:the blank group(N),the COPD group(B),and the acute exacerbation of COPD group(C),with only 8 rats in each group.Then the model of COPD with the improved smoke plus the tracheal drops of the lipopolysaccharide(LPS) was established.Then produce the kidney damage model with the method of the adenine gavage.Detect the expressions of the rat urine Cys-C mRNA with the RT-PCR method and the content of the matrix metalloproteinases-9(MMP-9) in lung homogenate with ELISA method.Results:Compared with the blank group,in each experimental group,the Cys-C mRNA expression were significantly increased(P0.01),and the COPD plus kidney damage group increased more than stable group(P0.05),while the simple renal impairment group increased(P0.05) more than the stable group;the level of MMP-9 in the lung homogenate increased significantly(P0.01),and the COPD plus kidney damage group increased more than stable group(P0.05).Conclusion:In the COPD disease,the lung infection and damage of functions have certain effects on the kidney function,and COPD can bring certain damage to the kidney.All of these point out the relationship between the kidney and lung in the course of COPD,and we should pay attention to the protection of the kidney in the COPD treatment process.
