仙连颗粒治疗激素依赖性特发性血小板减少性紫癜临床研究

摘　　要：目的:探讨仙连颗粒治疗激素依赖性特发性血小板减少性紫癜(ITP)的临床疗效。方法:将64例患者随机分为对照组32例和治疗组32例,对照组口服强的松片,治疗组在对照组基础上加用仙连颗粒;两组均以2个月为1个疗程,2个疗程后观察两组患者疗效和外周血象的改善情况,同时测定治疗前后血浆皮质醇(COR)浓度。结果:2个疗程后治疗组的有效率明显高于对照组,治疗后治疗组在外周血象的改善方面与对照组比较均有显著性差异(P<0.05)。同时发现治疗组激素依赖性ITP患者血浆COR治疗前后比较有显著性差异,对照组治疗前后无显著性差异。进一步研究发现,仙连颗粒对阴虚血热证患者的疗效优于气虚不摄证患者。结论:①仙连颗粒能改善激素依赖性ITP患者的临床症状、外周血象,具有升高血小板功效的仙连颗粒联用强的松治疗激素依赖性特发性血小板减少性紫癜疗效优于单用免疫抑制剂强的松。②阴虚血热证病程短,治疗较易,气虚不摄证病程长,治疗较难。Objective：To observe the Xianlian granule′s clinical effect on patients with steroid-dependent ITP.Methods：64 patients were randomly divided into the control group of 32 patients and the treatment group of 32 patients.The treatment group was given oral Xianlian granule and prednisone tablets at the same time;control group was treated with oral prednisone tablets and both groups regarded two months as a course of treatment.The efficacy of patients and the improved situation of the peripheral blood of the two groups were observed after two courses,the simultaneous determination of the concentration of plasma cortisol（COR） before and after treatment.Results：The results showed that the effective rate of treatment group after treatment were higher than control group.The difference between control group and treatment group in the clinical symptoms,melioration of hemogram and bone marrow phenomeno after treatment was significant（P0.05）.The levels of plasma cortisol concentration and plasma corticotropin releasing hormone concentration in patients with steroid-dependent ITP are significantly different in two groups.This showed the two treatments can resumption inhibit hypothalamic-pituitary-adrenal function.It was also observed that yin deficiency and heat in blood type was easier to treat than qi failing to control blood type.Conclusion：① Xianlian granule can improve the clinical symptoms of hormone-dependent patients with ITP and hemogram,suggesting Xianlian granule with elevated platelet efficacy combined with prednisone is superior to the single use of inhibitors prednisone in the treatment of hormone-dependent immune idiopathic thrombocytopenic purpura.② Yin blood heat syndrome has short duration and can be treated easier,while the syndrome of qi failing to restrict has longer duration and the treatment is more difficult.
