摘　　要：目的探讨多功能外固定架配合髓内钉治疗胫腓骨开放性骨折的疗效。方法对22例(男17例,女5例)胫腓骨开放性骨折患者一期采用多功能外固定架临时固定3～4周,二期拆除外固定改用髓内钉固定的治疗效果进行回顾性分析。结果 22例行3～48个月随访,21例胫腓骨开放性骨折均在12～24周内愈合,未发生骨折不愈合,无深部感染及骨髓炎发生,1例出现深静脉血栓形成并发肺栓塞死亡。结论多功能外固定架配合交锁髓内钉治疗胫腓骨开放性骨折,取得了满意疗效,具有治疗效果好、愈合时间短、术后并发症少等优点,为临床治疗胫腓骨开放性骨折提供了一种疗效肯定的治疗方法。Objective To explore the curative effect of interlocking intramedullary nail combined with multifunction external fixation apparatus in the treatment of open fracture of tibia and fibula. Methods 22 cases （17 males and 5 females） of open fracture of tibia and fibula were analyzed retrospectively. All the patients were treated with temporary external fixation for 3 to 4 weeks and treated with interlocking intramedullary nail after removal of multifunctional external fixation apparatus 3 weeks later. Results The average follow-up time was 18.5 months （3 - 48 months）. 21 patients were cured during 12 to 24 weeks, without nonunion, deep infection or osteomyelitis. One patient died of deep vein thrombosis complicated with pulmonary embolism. Conclusions Interlocking intramedullary nail combined with multifunction external fixation shows a desirable effect in the treatment of open fracture of tibia and fibula with advantages such as better effect of fixation, shorter healing time, and fewer postoperative complications, which provides an affirmed efficacy in the treatment of open tibiofibular fracture.
