声带息肉和声带小结的VHI分析

摘　　要：目的探讨声带息肉和声带小结嗓音障碍指数(VHI)的临床意义及其相关性。方法分别对声带息肉组(n=35)、声带小结组(n=29)及对照组(n=22)进行VHI调查,VHI调查包括功能(F)、生理(P)、情感(E)三个范畴,记录三方面得分及总分(TVH)分值。结果声带息肉组VHI的F、P、E、TVH平均值分别为9.57±7.62,18.86±8.91,8.97±8.77,36.83±21.32分,声带小结组VHI的F、P、E、TVH平均值分别为9.75±7.87,20.62±7.95,7.62±7.79,38.00±20.78分,正常组的VHI的F、P、E、TVH平均值分别为0.55±0.80,0.73±1.03,0,1.27±1.67分,声带息肉和声带小结的VHI平均值均高于对照组(P<0.01),声带息肉和声带小结VHI平均值比较,P>0.05;声带息肉和声带小结患者每日说话时间、病程与VHI之间无相关性(P>0.05);声带息肉的职业用声者VHI(39.48±22.67)和非用声职业(27.88±13.42)比较,P>0.05;声带小结的职业用声者的VHI平均值(42.35±22.11)和非职业用声者(31.83±17.82)比较,差异亦无统计学意义(P>0.05)。结论声带息肉和声带小结的VHI可主观反映患者嗓音障碍程度,每日说话时间、病程和职业不是VHI的影响因素。Objective To characterize the psychosocial impact of dysphonia on patients with vocal polyps or nodules with voice handicap index （VHI）. Methods VHI was administered to the vocal polyps group （35 cases）, the vocal nodules group （29 cases） and the control group （22 cases）. VHI scale included functional （F）, physiological （P）, emotional （E）, and TVH denoted the total sum. Results VIII scores for the vocal polyps group and the vocal nodules group were significantly greater than the control group （P〈0.01）. VHI scores had no statistically significant difference between the vocal polyps group and the vocal nodules group （P〉0.05）. No correlation was among VHI scores, speaking time per day and duration of current complaint （P〉0.05）. VHI scores had no significant difference between the professional and the non-professional voice users （P 〉0.05）. Conclusions VHI scale can be effectively used to evaluate the severity of voice disorder for the patients with vocal polyps or nodules. The speaking time perday, duration of current complaint and profession may not affect VHI.
