“双C”强化治疗下2型糖尿病夜间低血糖的相关因素分析

摘　　要：目的分析2型糖尿病患者在双C强化治疗模式下夜间低血糖的相关因素。方法 118例2型糖尿病患者采用双C治疗方案,提取动态血糖监测中当天9 pm～次日6 am的血糖数值,同时记录其空腹及餐后2 h的血糖、C肽、糖化血红蛋白及总胆固醇等因素,采用Logistic回归分析对可能造成夜间低血糖的相关因素进行统计分析。结果单因素条件分析结果显示,年龄、病程、体质量指数(BMI)、空腹C肽(FCP)、尿白蛋白(UA)、空腹血糖、晨起皮质醇是2型糖尿病低血糖发生的危险因素。多因素条件分析结果显示,年龄、病程、BMI、FCP、UA与2型糖尿病夜间低血糖发生有关。结论 2型糖尿病患者出现夜间低血糖与年龄、病程、BMI、FCP、UA有关,是多因素综合作用的结果。Objective To analyze the relevant factors of nocturnal hypoglycemia in patients with type 2 diabetes under dual C intensive treatment. Methods A total of 108 patients with type 2 diabetes were administered dual C treatment. The numerical value of blood glucose from 9 pm to 6 am every day was taken in continuousglucose - monitor system, and the factors such as blood glucose, C peptide, glycosylated hemoglobin and total cholesterol in fasting or 2 h after meals were recorded. Logistic regression analysis was adopted to analyze the relevant factors of nocturnal hypoglycemia in patients. Results The results of single - factor analysis indicated that age, course of disease, body mass index （BMI）, fasting C peptide （FCP）, urinary albumin （UA）, fasting blood glucose and morning eortisol were dangerous factors of nocturnal hypoglycemic occurrence in patients with type 2 diabetes. The results of multi - factor analysis demonstrated that age, course of disease, BMI, FCP and UA were related to the nocturnal hypoglycemic occurrence in patients with type 2 diabetes. Conclusion The occurrence of nocturnal hypoglycemia in patients with type 2 diabetes is associated with age, course of disease, BMI, FCP and UA, which results from the synthetic action of multi - factors.
