摘  要:目的研究终末期肾脏病患者经动静脉内瘘行血液透析对左室舒张功能的影响。方法纳入50例终末期肾病(ESRD)患者为研究对象。在动静脉内瘘(arteriovenous fistula AVF)成形术前行心脏超声检查,在患者完成第一阶段透析[平均(93.26±8.21)d]后行第二次心脏超声检查。结果患者年龄平均(60±15)岁,50%为男性。在第一阶段透析完成后,患者的E峰、A峰、E/A、Em、Am和Em/Am较透析前均差异无统计学意义,但E波减速时间及左房内径的增加均有统计学意义。透析前后肺静脉血流速度及右室功能几乎无变化。结论终末期肾病患者经AVF透析一段时间后,左右心室的舒张功能并未出现明显损害,但是左室收缩功能明显受损,同时左房内径增加。Objective To analyze the effect of hemodialysis by AVF on left ventricular diastotic function m patients with end-stage renal disease. Methods Fifty patients were included in the study. Before AVF was surgically created, the patients were evaluated by echocardiogram. After the first hemodialysis session [mean (93.26±8.21) days] the sec- ond echocardiogram were performed. Results Mean age of patients was (60±15) years and 25 patients were female. E peak, A peak, E/A, Era, Am and Era/Am were not significantly changed after maintenance hemodialysis, the decelera- tion time of E wave and left atrial diameter were significantly increased. There was no significantly change in pul- monary vein velocities and right ventricular functions before and after hemodialysis. Conclusion After maintenance hemodialysis treatment, left and right ventricular diastolic functions are not impaired, however left ventricular systolic functions were impaired.
