腰硬联合麻醉复合异丙酚在妇科腹腔镜手术中的临床应用

摘　　要：目的探讨腰硬联合麻醉复合异丙酚用于妇科腹腔镜手术的麻醉效果。方法选取2010年1月～2012年1月本院妇科80例要求行腹腔镜手术患者,随机分为腰硬联合麻醉(CSEA)组及气管内插管全身麻醉(GA)组,各40例,观察两种麻醉的麻醉效果及对患者呼吸、循环的影响。结果两组患者术中SpO2均维持在95%以上;CSEA组在T2时HR和RR均增快,MAP下降,与T0及同时间点GA组HR、RR、MAP相比差异有统计学意义(P<0.05);T4时MAP恢复至T0水平;GA组T3时MAP恢复至T0水平,而PaCO2值显著升高,与T1及同时间点CSEA组PaCO2值相比,差异有统计学意义(P<0.05)。结论 CSEA复合异丙酚用于妇科腹腔镜手术的麻醉,术中麻醉效果满意,且用药量少、经济实用、术后苏醒快,是安全可行的。Objective To investigate the anesthetic effect of spinal and epidural anesthesia combined with isopropylphenol in gynecological laparoscopic surgery. Methods Eighty patients who asked for laparoscopic surgery in Department of Gynecology of our hospital from January 2010 to January 2012 were selected and randomly divided into CSEA group （given combined spinal and epidural anesthesia） and GA group （given general anesthesia with endotracheal intubation）, respectively 40 cases. The anesthetic effect and the influence to respiration and circulation of patients were observed. Results The SpO2 of both groups＂ patients were kept above 95% during operation. At the time of T2, the HR and RR of CSEA group was accelerated, MAP decreased. Compared with FIR, RR, and MAP of GA group at the time of TO and same time point, there were statistical differences （P 〈 0.05）. At the time of T4, MAP of CSEA group returned to the level of TO. At the time of T3, MAP of GA group returned to the level of TO, but the PaC02 significantly elevated, compared with the PaCO2 of CSEA group at the time of T1 and same time point （P 〈 0.05）. Conclusion For gynecological laparoscopic surgery, spinal and epidural anesthesia combined with isopropylphenol has satisfied anesthetic effect during operation, use less medication, is economic and practical, has faster analepsia after operation, and is safe and feasible.
