甲基强的松龙联合静脉丙种球蛋白治疗重症血小板减少性紫癜的临床研究

摘　　要：目的探讨甲基强的松龙联合静脉用丙种球蛋白治疗重症血小板减少性紫癜的临床疗效。方法选择2006～2011年南阳市中心医院收治的重症血小板减少性紫癜患者49例,分为对照组及治疗组。对照组给予甲基强的松龙15～30 mg/(kg·d),连用5 d;治疗组在给予甲基强的松龙的同时联合静脉用丙种球蛋白0.4 g/(kg·d),连用5 d。两组在用药3、5 d后观察治疗效果,比较两组血小板回升时间及出血停止时间。结果用药3、5 d后治疗组疗效均优于对照组,差异有统计学意义(P<0.05);治疗组血小板回升时间及出血停止时间均短于对照组,差异有高度统计学意义(P<0.01)。结论甲基强的松龙联合丙种球蛋白可快速明显地提升重症血小板减少性紫癜患儿的血小板到安全范围,并可减少单一用药副作用,是治疗重症血小板减少性紫癜的有效方法,值得在临床推广。Objective To discuss the therapeutic effects of Methylpredinsolong combined with intravenous gammaglobulin（IVIG） for severe idiopathic thrombocytopenic purpura（ITP）.Methods 49 cases of patients with severe ITP in the central hospital of Naryang City from 2006 to 2011 were selected and divided into the treatment group and the control group.The control group was treated with Methylpredinsolong 15-30 mg/（kg·d） for 5 d.The treatment group was treated with Methylpredinsolong combined with IVIG 0.4 g/（kg·d） for 5 d.Clinical effectiveness 3 and 5 d after treatment of the two groups were observed,and the platelet increasing time and blood stop time of the two groups were compared.Results 3 and 5 d after treatment,the curative effects of the treatment group were all better than those of the control group,the differences were statistically significant（P 0.05）.The platelet increasing time and blood stop time in the treatment group were all shorter than those of the control group（P 0.01）.Conclusion Methylpredinsolong combined with IVIG can quickly increase the platelet count to normal level,and decrease the drug side effects of single drug,which is the effective and safe treatment method for severe ITP.It is worthy of clinical application.
