189例良性阵发性位置性眩晕患者手法复位治疗

摘　　要：目的：总结分析良性阵发性位置性眩晕（BPPV）患者应用手法复位治疗后的短期疗效。方法：2011年7月-2012年7月间对189例BPPV患者根据受累半规管的不同分别采用Epley管石复位法和Barbecue翻滚复位法进行治疗，并于1周后复查，随访3个月。结果：单侧后半规管BPPV患者157例，单侧水平半规管BPPV患者24例，单侧上半规管BPPV患者3例，同时性双侧后半规管BPPV患者3例，同时性后半规管与上半规管BPPV患者1例，同时性后半规管与水平半规管BPPV患者1例。手法复位1周后总治愈率为87．8％（166／189），其中后半规管BPPV为89．8％（141／157），水平半规管BPPV为79.2％（19，24）．上半规管BPPV为66．7％（2／3），混合性半规管BPPV为80％（4，5）。观察随访3个月后总治愈率为95．8％（181／189）。结论：Epley管石复位法和Barbecue翻滚复位法为BPPV的首选治疗方法。Objective：To review positioning therapy results with benign paroxysmal positional vertigo （BPPV）. Methods：A total of 189 cases of BPPV treated with particle repositioning maneuver and rolling maneuver therapy from July 2011 to July 2012 were retrospectively studied.Results：One hundred and fifty-seven patients had posterior semicircular canal BPPV.Twenty-four patients had horizontal semicircular canal BPPV.Three patients had superior semicircular canal BPPV.Three patients had bilateral posterior semicircular canal BPPV.One patient had combined posterior and superior semicircular canal BPPV.One patient had combined posterior and horizontal semicircular canal BPPV.When treated with panicle positioning maneuver,the initial success rate was 87.8%,89.8% for posterior semicircular canal BPPV,79.2% for horizontal semicircular canal BPPV ,66.7% for superior semicircular canal BPPV and 80% for combined semicircular canal BPPV.After three months following up,the success rate was 95.8%.Conclusion：Particle repositioning maneuver and roiling maneuver therapy can be chosen as the first choice to patients with BPPV.
