HSP90和GRP78与非小细胞肺癌完全切除术后吉西他滨化疗预后的相关性研究

摘　　要：【目的】探讨热休克蛋白90（HSP90）和葡萄糖调节蛋白78（GRP78）在ⅢANz期非小细胞肺癌（NSCLC）中的表达及其与吉西他滨辅助化疗的相关性。【方法】免疫组化方法检测HSP90、GRP78在134例手术完全切除ⅢA—N2期NSCI．C中的表达，分析HSP90和GRP78的表达与临床、病理及患者预后的关系。【结果】ⅢAN2期NSCLC中HSP90和GRP78阳性表达率分别为64．9％和61．2％；HSP90和GRP78的表达与吉西他滨化疗明显相关，HSP90和（或）GRP78表达阴性者在含吉西他滨化疗组有更长的无瘤生存期（P〈0．05）和总生存期（P〈O．05）。【结论】HSP90表达阳性、组织病理分化低、全肺切除是影响ⅢA—N2期NSCLC预后的危险因子；HSP90和GRP78的表达与铂类化疗相关，表达均阴性者预后较好。[Objective]To explore the expression of HsPg0 and GRP78 in non-small cell lung cancer（NSCLC） with stage ⅢA-N2, and their correlation with gemcitabine adjucant chemotherapy. [Methods]Immunohistochemical analysis was used to determine the expression of HSP90 and GRP78 in completely resected NSCLC with stage IlIA-N2. The corre- lation of the expression of HSP90 and GRP78 with clinicalpathology and prognosis were analyzed. [Results] Among 134 cases, The positive expression rates of HSP90 and GRP78 in NSCLC with stage ⅢA-N2 were 64.9% and 61.20%, re- spectively. The expressions of HSP90 and GRP78 were associated with gemcitabine chemotherapy. Patients with the neg- ative expression in gemcitabine adjuvant chemotherapy group had longer disease-free survival（DFS）（ P = 0. 000, P = 0. 018） and overall survival（OS） （ P 〉0. 000, P =0.011）. The patients with the negative expression of both HSP90 and GRP78 in gemcitabine adjuvant chemotherapy group had longer DFS（ P = 0. 000） and OS（ P = 0. 000）. [Conclusion] HSP90-positive expression, low histological differentiatation and whole lung resection are the risk factors infuencing the prognosis of NSCLC with stage ⅢA-N2. The expressions of HSP90 and GRP78 are associated with gemcitabine chemo therapy. Patients with the negative expression of both HSP90 and GRP78 have better prognosis.
