Ⅰ期平片无张力疝修补术结合切口皮下负压引流在老年急性绞窄性腹股沟疝的临床应用

摘　　要：目的探讨应用Ⅰ期平片无张力疝修补术结合切口皮下负压引流治疗老年急性绞窄性腹股沟疝的处理要点,并评价其疗效。方法回顾性分析自2006年1月至2012年3月我科施行的应用Ⅰ期平片无张力疝修补术结合切口皮下负压引流治疗25例老年急性绞窄性腹股沟疝的临床资料。结果全组无死亡病例,术后均痊愈出院。其中术后出现阴囊积液1例,切口感染5例,切口延迟愈合1例,右腹股沟区胀痛不适1例。随访3～73个月,无疝复发。结论对于老年急性绞窄性腹股沟疝患者,注意加强围手术期处理,术中严格无菌操作,肠切除者放置腹腔引流管,应用Ⅰ期平片无张力疝修补术结合切口皮下负压引流治疗是一种简便、可行的有效治疗方法,值得临床推广应用。Objective To explore the processing points of the clinical application of stage-I tension-free herniorrhaphy with polypropylene mesh and subcutaneous negative pressure drainage tube on senile patients with acute strangulated inguinal hernias and to evaluate its efficacy. Methods From January 2006 to March 2012 ,the clinical data of 25 senile patients with acute strangulated inguinal hernias underwent one-stage tension-free herniorrhaphy with polypropylene mesh and subcutaneous negative pressure drainage tube were analyzed retrospectively. Results There was no death in whole group, all postoperative cases were fully recovered. But 1 case was complicated with scrotal hydrocele,5 cases with infection of incision, 1 case with incision delayed union, 1 case with gas pains in right inguinal region. All cases were followed-up from 3 months to 73 months after operation, and no secondary recurrence was found. Conclusion With regard to senile patients with acute strangulated inguinal hernias, provided strengthening the perioperative management, abiding by intraoperative aseptic processing strictly, and placing celiac drainage tubes for those who undergo intestines, excision, the clinical application of stage- I tension-free herniorrhaphy with polypropylene mesh and subcutaneous negative pressure drainage tube is a convenient, feasible and effective therapeutic method, indicating its value in wide practice.
