专人陪护联合水针镇痛在自然分娩中的临床应用

摘　　要：目的提高自然分娩率,降低剖宫产率。方法将120例正常初产妇随机分为观察组和对照组各60例。观察组给予专人陪护联合水针镇痛(1%利多因+0.9%生理盐水1∶1稀释液,作皮内注射);对照组采取普通陪伴并不用水针镇痛。结果观察组疼痛评分,剖宫产率,阴道助产率,产后2 h出血量,胎儿宫内窘迫和新生儿窒息发生率显著低于对照组(P<0.05或P<0.01),观察组产妇第一产程、第二产程、第三产程及总产程时间均短于对照组(均P<0.01)。结论专人陪护联合水针镇痛可以减轻产妇疼痛,提高顺产率,缩短产程,减少产科发病率和围产儿发病率等,促进自然分娩。Objective To improve the natural delivery rate and reduce the rate of cesarean section.Methods All 120 cases of primipara were randomly divided into observation group and control group with 60 cases in each.The observation group received the full-time nursing and anesthesia with lidocaine-distilled water（1% lidocaine and 0.9% 1∶1 saline diluent,intradermal injection）;The control group received the general accompany and did not take anesthesia with lidocaine-distilled water.Results In the observation group,the pain score low as compared to the control,the rate of abdominal delivery and operative vaginal delivery decreased,the blood volume 2 hours after the delivery was low,the rate of fetal distress and neonatal asphyxia decreased as compared with the control group（P0.05 or P0.01）.The first,second,third and total stage of labor time in the observation group was shorter than that in the control group（P0.01）.Conclusion The full-time nursing combined with anesthesia with lidocaine-distilled water can release the pain of puerpera woman,increase the rate of vaginal delivery,shorten the birth process,reduce the incidence of obstetric diseases and morbidity of perinatal infants,and promote natural delivery.
