摘 要:目的分析男性精浆弹性蛋白酶水平对体外受精胚胎移植术(invitrofertilizationandembryotransfer,IVF-ET)中相关实验室参数及临床结局是否存有影响。方法筛选出本生殖中心2012年1月至5月56个IVF治疗周期,根据男方精浆弹性蛋白酶含量分为正常组(弹性蛋白酶〈290ng/ml)、隐性感染组(弹性蛋白酶290~1000ng/m1)和感染组(弹性蛋白酶〉1000ng/ml),比较IVF治疗周期中3组的精子密度、精子活力、卵子受精率、正常受精率、卵裂率、优质胚胎率、生化妊娠率、临床妊娠率及胚胎种植率等指标之间有无差异。结果3组IVF治疗周期在精子密度、精子活力、受精率、正常受精率、卵裂率、生化及临床妊娠率等指标方面差异无统计学方法;正常组胚胎种植率(35.9%)高于隐性感染组(27.6%)和感染组(26.8%),但差异亦无统计学意义(P〉0.05);在优质胚胎率方面,正常组(48.8%)高于隐性感染组(34.3%)和感染组(35.2%),两组感染组与正常组比较,差异存有统计学意义(P〈0.05),但隐性感染组和感染组间差异无统计学方法。结论弹性蛋白酶是反映男性生殖系统感染的监测指标之一,与IVF治疗周期中某些参数存有相关性,其对男性生殖能力或许存有潜在影响。Objective To explore the relationship betweenthe level of seminal plasma elastase and major parameters in vitro fertilization and embryo transfer. Methods A retrospective study on 56 couples treated by IVF in our center from January to May of 2012 was carried out in the study. According to levels of seminal plasma elastase, 56 cycles were divided into three groups such as the normal group, the recessive infection group and the infection group. The quality of sperm, fertilization rate, normal fertilization rate, cleavage rate, good-quality embryo rate, bio- chemical pregnancy rate, clinical pregnancy rate and implantation rate were comparatively analyzed among three groups. Results There were no significant differences in the quality of sperm, fertilization rate, normal fertilization rate, cleavage rate, biochemical pregnancy rate and clinical pregnancy rate among three groups. The implantation rate was higher in the normal group (35.9%) than those of the other two groups (27.6% and 26.8%), but having no statistical differences (P〈0.05). The good-quality embryo rate was significantly higher in the normal group (48.8%) than that inthe recessive infection group (34.3%) and the infection group (35.2%) (P〈0.05). Conclusion Seminal plasma elastase might be a biomaker of male reproductive tract inflammation and has negative impact on some parameters in the cycles of in vitro fertilization and embryo transfer. It might affect male fertility potentially.
