摘 要:目的:通过对家兔离体心脏缺血再灌注模型制作方法的改进,延长家兔离体心脏存活时间并提高其存活状态,为研究离体心脏缺血再灌注提供更为稳定的模型。方法:健康新西兰家兔20只随机分为对照组与实验组,每组10只;通过改进实验方法与优化剖取家兔心脏术式操作,记录两组离体心脏复跳和从复跳到稳定跳动时间、比较两组离体心脏的存活时间及状态。结果:改进组优化手术操作完全暴露家兔心脏、减少了心肌机械性损伤,用改进台氏液冲洗离体心脏,缩短了从心脏取出到主动脉挂于灌流装置上该过程的时间,从而缩短了心脏从离体到再灌注此段的缺血时间;灌流液的温度从30℃缓慢升至37℃,此过程避免了离体心脏从4℃环境直升为37℃的初期供氧与灌流液温度条件变化而带来的心肌内环境紊乱,加之在灌流装置中增加了心脏保温罩、不断更换以95%O2+5%CO2氧饱和气化的新鲜台氏液,以共同维持离体心肌的正常生理功能。结果两组对比,改进组家兔离体心脏复跳时间和从复跳到稳定跳动时间比对照组缩短、明显显示其稳定有节律跳动时间延长(P<0.01)。结论:本改进法可使家兔离体心脏复苏快、且能持续有节律稳定跳动7 h左右,达到延长家兔离体心脏存活时间的目的,该模型可适用于专业实验教学和科学研究。Objective: To establish a method to prolong the survival time of rabbit heart ischemia-reperfusion model in vitro.Methods: 20 New Zealand rabbits were evenly and randomly divided into control group and tested group.The control groups were performed with classic surgery method with 4℃ Tyrode's buffer perfusion.While tested group was operated with the improved rabbit cardiac surgery,which included the uncovered cardiac surgery,elevating the temperature of Tyrode's cardiac perfusion buffer from 4℃ to 30℃ and consistently renewing the gas of CO2-O2 in Tyrode's buffer.The recovery time of the operated cardiac rebeating and time from the early cardiac rebeating to the normal cardiac beating of the two groups were accounted for.Results: Comparing with the classical surgery,the improved surgery can significantly shorten the isolated rabbit cardiac rebeating time and the recovery time from unstable to the normal cardiac beating(P 0.01).Conclusion: Our improved methods will shorten the cardiac ischemia time and improve the recovery cardiac rebeating time from cardiac ischemia-reperfusion and the time from the rebeating to the normal beating.Our results demonstrate that improved operation system can increase the rabbit survival ratio under the ischemia-reperfusion condition.
