摘  要:目的探讨超声三维斑点追踪成像技术评价先天性心脏病患者左心室整体收缩功能的临床应用价值。方法应用超声三维斑点追踪成像技术测量并比较82例先天性心脏病及55例对照组的左室整体纵向收缩峰值应变(LVGLS)、左室整体径向收缩峰值应变(LVGRS)、左室整体环向收缩峰值应变(LVGCS)。结果先天性心脏病(CHD)组LVGCS、LVGLS、LVGRS均较对照组增高(P均<0.05),LVGCS、LVGLS、LVGRS与左室射血分数(LVEF)正相关,且以动脉导管未闭(PDA)组的LVGRS和房间隔缺损(ASD)组的LVGCS显著。结论先天性心脏病患者早期左心室整体三维形变能力增强,超声三维斑点追踪成像技术可发现CHD患者心功能的变化,三维斑点追踪技术可客观的评价左心室整体收缩功能,为评价左心室功能提供一种新方法 。Objective To assess left ventricular global systolic function in patients with congenital heart disease with 3D speckle tracking. Methods 82 patients with congenital heart disease were enrolled in this study and 55 normal subjects were selected as control groups. LV glohle longitudinal peak systolic strain (LVGLS), radial peak systolic strain (LVGRS), circumferential peak systolic strain (LVGCS) were measuresd in all subjects by 3DSTE and corn pared between groups. Results Compared with CHD group, I.VGLS,LVGRS,LVGCS were significantly increased (P〈0.05 for all). LVGCS, LVGLS and LVGRS were positive correclated with ejection fraction (LVEF) in CHD group. LVGRS in PDA group and LVGCS in ASD group were more significant. Conclusions LV global 3D strain in creases in patients witn CHD in early period. 3DSTE could evaluate the early change of heart function in patients with CHD. 3DSTE could evaluate LV global systolic function noninvasively and objectively, which prvide a new method to evaluate LV function.
