摘　　要：目的分析左房异构胎儿心血管及伴随心外畸形声像特征,探讨产前超声对左房异构的诊断价值。方法回顾19例左房异构胎儿产前超声资料,与胎儿尸检、外科手术结果对比,分析左房异构胎儿声像学特征。结果本组左房异构胎儿合并房室间隔缺损12例,单心房5例,右室双出口6例,流出道梗阻性病变7例,永存左侧上腔静脉5例,心动过缓4例,部分型肺静脉异位引流1例,内脏异位7例。心外畸形为静脉导管缺如4例,肠梗阻性病变3例,胆道闭锁2例,脐膨出1例,持续性左脐静脉1例,胼胝体缺如1例。多脾7例,右侧脾脏3例。结论左房异构胎儿多合并房室间隔缺损、流出道梗阻性病变、右室双出口及内脏异位症;心外畸形对产前诊断提供的特异性信息较少。Objective To describe the prenatal ultrasonographic findings in the fetus with Left atrial isomerism. Methods The prenatal ultrasonographic records of nineteen fetuses with Left atrial isomerism between 2008 and 2012 were reviewed. A detailed anatomical survey to evaluate cardiacvescular and extracardiac anomalies was performed in all cases. We compared antenatal and postnatal diagnoses and analyzed the sonograms characteristic of these cases. Re- suits There were 19 cases with Left atrial isomerism. Associated cardiac defects included complete atrioventricular septal Defect, common atrial, double outlet right ventricle, ventricular outflow tract obstruction, persistent left supe- rior vena cava, atrioventricular block and viscerocardiac heterotaxy. Extracardiac anomalies included 4 absence of duc- tus venosus, 3 intestinal obstruction, 2 biliary atresia and 1 omphalocele, persistent left umbilical vein, agenesis of the corpus callosum, respectively. There were 7 cases with asplenia and 3 cases with right spleen in all cases. Conclusions Associated cardiac defects of left atrial isomerism included complete atrioventricular septal defect, double outlet right ventricle, ventricular outflow tract obstruction and viscerocardiac heterotaxy while extracardiac malformations offer only minor diagnostic pointers.
