摘  要:目的建立新西兰大白兔高血压的动物模型,应用速度矢量成像技术(VVI)对高血压兔心脏局部及整体功能的动态变化进行分析,探讨高血压心脏的动态演变特点。方法新西兰大白兔40只,分为手术组(25只)和假手术组(15只),手术组和假手术组兔分别于术前、术后即刻、术后2、4、8周测量血压及腹主动脉内径和流速。所有实验动物于手术前、术后2、4、8周行超声心动图检查,并应用VVI分析收缩期峰值应变(Ss)、收缩期峰值应变率(SRs)、舒张期峰值应变率(SRd)。结果手术组兔实验过程中腹主动脉缩窄程度未见明显变化。术后血压明显增高。左室后壁厚度(LVPW)及室间隔厚度(IVS)术后4周均增大,有显著性差异,左室射血分数(EF)及左室短轴缩短率(FS)术后8周明显下降。Ss、SRs、SRd术后2周开始至术后8周均可见逐渐减低。结论 VVI能够无创性早期检测高血压所致心功能损害,同时在心功能进一步损害时能够定量评价心肌功能损害的程度。Objective To make hypertension model with New Zealand white rabbits. To assess regional myocar- dial function and global function of rabbits with hypertension by velocity vector imaging. To explore dynamic change of heart function in hypertention. Methods Forty New Zealand white rabbits were divided into operative group(n= 25) and sham operation group(n=15) . The diameter, velocity of abdominal aortic and blood pressure were measured in all rabbits at pre-operation and pro-operation instantly,2 weeks,4 weeks and 8 weeks. Routine echocardiography and Ss,SRs,SRd by VVI were analysed in all rabbits at pre-operation and pro operation instantly, 2 weeks, 4 weeks, 8 weeks. Results The coarctatic degree of abdominal aorta did not change over time. Blood pressure significantly in creased after operation. The thickness of LVPW and IVS increased significantly 4 weeks post-operative. 8 weeks after operation, EF and FS significantly decreased. In operation group, Ss, SRs, SRd decreased gradually within 8 weeks. Conclusions VVI is able to manifest myocardia damage in early stage, evaluate the myocardial function quantitatively when cardiac function further damaged.
