妊娠期糖尿病患者血清网膜素水平与糖、脂代谢指标的相关性研究

摘　　要：目的:探讨妊娠期糖尿病(GDM)患者血清网膜素(Omentin)水平的变化及其与糖、脂代谢指标的相关性。方法:采用酶联免疫吸附法检测35例孕前肥胖的GDM孕妇(A组)、32例孕前非肥胖的GDM孕妇(B组)、36例正常健康孕妇(C组)的血清Omentin水平;同时测定所有受试者的糖、脂生化指标,并计算胰岛素抵抗指数(HOMA-IR)。结果:(1)GDM孕妇三酰甘油(TG)、空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)水平均高于正常对照组,高密度脂蛋白(HDL-C)水平低于正常对照组(P均<0.05);(2)3组孕妇血清Omentin水平A组<B组<C组,两两比较差异显著(P<0.01);(3)血清Omentin水平与HDL-C呈正相关,与TG、FPG、BMI、FINS及HOMA-IR呈负相关(P<0.05);(4)多元逐步回归分析显示,FINS、FPG、孕前BMI是影响血清Omentin水平的重要因素(P<0.05)。结论:GDM孕妇存在糖、脂代谢异常及胰岛素抵抗,其血清Omentin水平降低,伴有肥胖的GDM孕妇代谢异常程度及胰岛素抵抗程度更重,其血清Omentin水平更低。Omentin水平降低可能参与了GDM的发生、发展。Objective：To investigate the change of serum omentin levels and the relationship among serum omentin levels and glucose,lipid metabolic indicators in GDM. Methods： Serum omentin levels were measured by enzyme-linked immuno sorbent assay （ELISA） in 35 cases of GDM with obesity （group A） ,32 cases of GDM with normal weight （group B） ,36 cases of normal weight pregnant women （ group C ）. Meanwhile, FINS, FPG, TC, TG, HDL-C, LDL- C were measured. Insulin resistance was estimated using the homeostatic model assessment-insulin resistance （HOMA-IR） index. Results： （ 1 ） TG, FPG, FINS and HOMA-IR levels in GDM were higher and HDL-C level was lower than those in the control group （P〈0.05）. （2） Serum omentin level was higher in group B than that in group A, and serum omentin level was the highest in the group C （P〈0.01）. （3）Positive correlation was showed between omentin and HDL-C. There was significant negative correlation between omentin levels and TG, FPG, BMI, FINS levels and HOMA-IR（P〈0. 05）. （4）In a multiple stepwise regression analysis, FINs, FPG, and prepregnant BMI were independent variables of omentin （P〈0.05 ）. Conclusion： GDM is characterized by glucose, lipid metabolic disturbance and insulin resistance. Serum omentin level is decreased in the GDM. The metabolic disturbanc and the insulin resistance are obesimore serious, and the level of serum omentin is more lower in the GDM accompanied with obesity. Reducing omentin level may be involved in the occurrence and development of GDM.
