摘  要:目的:探讨Ⅰ型牛胶原蛋白生物膜预防屈指肌腱损伤术后粘连的疗效。方法:选取符合诊断标准的屈指肌腱损伤患者72例,随机分为对照组和治疗组各36例。两组均采用改良的Kessler肌腱缝合法修复肌腱,治疗组用Ⅰ型牛胶原蛋白生物膜包绕吻合肌腱断端,对照组采用同一方法修复但未使用Ⅰ型牛胶原蛋白生物膜,充分止血后关闭伤口。术后第4周、8周分别对两组伤指的总主动活动度(TAM)测定,比较其差异性并行统计学分析。结果:所有病例均获得随访,术后两组伤指均有不同程度的肿胀、疼痛,所有病例伤口均一期愈合,无1例感染。①术后第4、8周,治疗组与对照组比较,肌腱总主动活动度测定结果比较差异有统计学意义(P<0.05)。②术后第8周末伤指总主动活动度测定,治疗组优良率为83.34%,对照组优良率为50%,治疗组优良率明显高于对照组。结论:Ⅰ型牛胶原蛋白生物膜应用于临床预防肌腱粘连疗效确切,显著促进手功能的恢复,且其操作方法简单、安全性较高,为防止组织粘连特别是肌腱领域开辟了新途径。Objective:To study the efficacy of type I bovine collagen membrane in preventing flexor tendon adhesion after injury. Methods:72 patients in conformity with the diagnostic criteria of flexor tendon injury were randomly divided into control group and treatment group, 36 patients in each group. The patients of both groups were all repaired by modified Kessler suture method. Type I bovine collagen membrane was used to wrap tendon repairing site in treatment group while it was not used in control group. 4 and 8 weeks later, the total active movement (TAM) of injured finger was measured and the data was compared statistically between the two groups. Results:All the patients were followed up. All the injured fin- ger had different levels of swelling and pain after surgery, and the surgical incisions healed primarily without infection in all patients. (1) 4 and 8 weeks later, the total active movement of treatment group was better than that of the control group. There was significant difference between the two groups (19 d0.05). (2) 8 weeks later, the total active movement of injured fingers were measured. The excellent effective rate was 83.34% in the treatment group while 50% in the control group. Concluslon:Type I bovine collagen membrane has reliable efficacy in preventing flexor tendon adhesion and significantly promote the recovery of hand function. The technique has been shown to be safe easy to perform . It is a new way of preventing post-traumatic tissue adhesion especially tendon adhesion.
